@detroitmike I have the same ISO result as you and just had my MRI, so I don’t know my pi rads yet. With a 2, did they still do a biopsy?

With that high an ISOPSA, you should be able to light up a PSMA PET scan because something is causing that number. Your result is high, IsoPSA >10 is associated with a sevenfold higher risk of csPCa (clinically significant prostate cancer).
You could do a PSMA Pet test before a biopsy just to see if there’s cancer somewhere else.
Would be interesting to see your actual PSA.
