Hypocellularity of 10-20% and moderate fibrosis in 25 year old?
Hi all, I recently had a bone marrow biopsy performed with the following results. The pathologist stated that I had a hypocellularity of 10-20% with MF 1-2/3, and signs of “evolving bone marrow failure.” There were no signs of overt cancer, blasts 1-2%. Neo comprehensive Myeloid Panel was negative, but that did not include FISH tests. I’ve done a lot of research because my follow up is not for a couple of weeks, but based on what I’ve done it might be between hypocellular MDS or aplastic anemia.
WBC - 3.35 RBC - 4.39 HGB - 13.4 HCT - 40.0 MCV - 91.1 RDW - 12.3 Plt - 229
Differential: immature grans 0.3%, neu 48.3%, lymphs 41.8%, monos 8.7%, eos 0.3%, basos 0.6%.
Peripheral blood:
Erythrocytes: Normocytic, normochromic RBCs without anemia, no polychromasia or nucleated RBCs present
Anisocytosis and poikilocytosis: Not prominent, mild rouleaux formation.
Leukocytes: Leukopenia with borderline absolute neutropenia. No atypical lymphocytes or circulating blasts
identified.
Platelets: Adequate

IMMUNOHISTOCHEMISTRY
STAIN RESULT:
CD34: Highlights scattered blasts, 1-2%.
CD117: Positive in the blasts and a few scattered mast cells.
CD31: Positive in scattered megakaryocytes
E cadherin: Positive erythroid precursors small islands
MPO: Positive in slightly decreased myeloid precursors with ME ratio about 1-2: 1.
CD3/CD20: Positive in mildly increased T/B lymphocytes.
CD138: Positive in scattered plasma cells, less than 5%.
CD79 a/Pax 5:Positive in B lineage cells with slightly increased hematogones.

Final Diagnosis
PERIPHERAL BLOOD:
- LEUKOPENIA WITH BORDERLINE ABSOLUTE NEUTROPENIA AND LYMPHOCYTOPENIA, NO
ATYPICAL LYMPHOCYTES OR CIRCULATING BLASTS IDENTIFIED.
- NORMOCYTIC NORMOCHROMIC RBCS WITH MILD ROULEAUX FORMATION, NO POLYCHROMASIA
OR NUCLEATED RBCS PRESENT.
- ADEQUATE PLATELETS.
BONE MARROW, ASPIRATE, CLOT AND CORE BIOPSY:
- HYPOCELLULAR BONE MARROW (10-20% CELLULARITY) FOR AGE WITH RELATIVELY
DECREASED MYELOID PRECURSORS, ERYTHROPOIESIS AND MEGAKARYOCYTES.
- NO INCREASE IN BLAST POPULATION.
- FOCALLY MODERATE MYELOFIBROSIS (MF 1-2/3).
- ADEQUATE STAINABLE STORAGE IRON PRESENT, NO RING SIDEROBLASTS IDENTIFIED.

Hi @meme67. While I know you’d like to get feedback for your test results, members in Connect aren’t able to diagnosis, interpret test results or offer treatment for medical conditions. Speculating on what these results could mean could be giving you information that may not even be relevant to your health.
I know how frustrating it is to have test results pop up in the patient portal and having to wait for the appointment with the doctor to go over the findings.
What were your symptoms which lead to needed a bone marrow biopsy?
Hi @meme67, have you had a chance to discuss the results of the bone marrow biopsy with your doctor in the meantime? Any update?
The hematologist, who wasn’t very friendly at all, said that there was no findings of cancer. My numbers had improved/normalized at the appointment, so he said basically he is not certain why the findings showed as hypocellular and fibrosis, but that he is basically not concerned regarding it because my numbers normalized. I am not sure whether to trust this or seek another opinion.
Hi all. I wanted to get other's perspective, I know no one here can provide medical advice, but really I'm just looking for your personal experiences or if someone has gone through something similar. I was having some strange symptoms - shortness of breath, chest pain with exertion. My blood counts were low, the did a bone marrow biopsy and found that my cellularity was at 10-20%
(I am 25), and that there was "moderate" fibrosis. The pathologist said that I had "evolving bone marrow failure." When I went to my follow up appointment, my counts had stabilized. I had met with the PA previously who was nice, but he was very rude and had horrible bedside manner. He basically said in so many words that he doesn't know what is wrong, there is something causing the bone marrow to react this way, but was no longer concerned about it because my blood counts stabilized. He said there was no need to follow up. Should I take this, or should I get a second opinion?

It can be unsettling when lab tests show abnormal values and then normal values again. It's hard to know what to trust. Will you be monitored tested again in a few months? What symptoms led to the testing in the first place? Are you still having symptoms?
