Trouble with vacuum pumps
Despite surgery and radiation and now one more hormone shot , I bought a vacuum pump 2 months ago, I’m totally frustrated with it , I can get an erection close in size before surgery but not hard enough for sex , any suggestions, I’m not ready for implant or injection but will be after my second hormone shot wears off in 3 months

I understand the frustration. I have been a proponent of the wait and see experience but I'm beginning to get more concerned. I have always thought that if we find a cancer, maybe it could be treated with radiation and possibly removed. It's a shame to start the miserable life of ADT without knowing where the cancer is located.
I [suppose I should be] well, actually, I am grateful that sexual performance is currently my major symptom of PC! Although I do have an aching in my hip that I wonder about...."hopefully" just arthritis?! Like I say, symptom of PC. Currently I'm having success using sildenafil before encounters, and a VED (vacuum erection device) for penile smooth muscle exercise when I am not sexually active. I do not use the VED for encounters. I tried various things before this, and spent about 2 yrs doing injections. I wish I had started using the VED sooner, but that was yesterday. The RestoreX tension device did not work as well as the VED does, and the sildenafil and tadalafil gave me no results and major side effects for a long time. Now I get some results with the sildenafil and manage the side effects fairly well.
How I use the VED: Over time, I have discovered that cycling the VED between reasonable (significant, but still comfortable) vacuum and no vacuum for 10-20 minutes is the most effective. Increased vacuum or decreased time do not help. Since it is for exercise, I only use it when the penile muscles are otherwise staying flacid.
As far as spontaneous nocturnal erections, I do sometime get a hint from time to time. It may be correlated with having used sildenafil within the prior 24 hours. It also may correlate with being over tired and not being able to sleep--so maybe it is also happening but not noticeable while I'm asleep? I did consider a ring that gives a record of arousal on a smart phone, but ultimately decided that additional data wasn't going to change much.

Google “online ED meds” and you’ll get numerous choices of where to buy online. Most will prescribe the meds without an in person meeting.
Had a great sex life until RP 10/21. Unable to get erections with pills. Used the pump with mixed results. Able to get erection and slide the ring onto the base of my penis to maintain the erection. Somewhat uncomfortable. Also, flopped around as it wasn't stiff beneath the ring.
So, I tried Trimix. Like most men, I was reluctant to put a needle (no matter how small) in my penis but the urge to have sex again won out. After finding the right dosage (a couple of times) I had the firmest section I have had in decades. Main issue (some would as "Is this a problem?!") is it took 2-3 hours for the erection to go down. I would Duse the pump for a few minutes before injecting the Trimix. Alternate sides and injection site.
Additionally I take generic Cialis, 5mg, daily. The docs warn against using any stronger oral ED meds in conjunction with the Trimix as it can cause priapism (prolonged erections requiring medical intervention, i.e putting a larger needle in the penis to drain the blood, don't want that!).
A year after my RP I had triple therapy for oligo metastatic disease and was on Lupron for a year. That killed my libido. Also had whole pelvic radiation after triple therapy. My MO said the pelvic radiation would result in permanent ED. Amazingly, after 2 years of no testosterone (never came back after Lupron discontinued) my MO put me on TRT (PSA undetectable >2 years) and I started having spontaneous nocturnal erections and then with foreplay. Never say never.
Hi Doc , would like to know more about your miracle comeback, what is TRT ? Like you I had a great sex life until my surgery and radiation and soon a second Lupron shot . Will Trimex work while on Lupron? I’m climbing the walls whenever my younger and attractive wife takes her clothes off , I’m ready to bite my fingers off some days . Gave up on 100 mg of Viagra and the pump and depending on the Lupron ready to try the needle and last resort an implant. How old are you ? I was stunned at the 2-4 hrs of your errections , I would kill for that right now , how did you survive 2 yrs before you can have normal sex without the trimex I assume? Good for you , sounds like you’re as stubborn as I am , my wife and oncologist think I put too much emphasis on sex but it’s so much more than that , it’s what has kept me 10 yrs younger than I am

TRT is Testosterone Replacement Therapy.
I would strongly encourage you to try Trimix injection, especially before implant surgery. It absolutely works while on Lupron. The Urologists suggest using it no more than 3x a week or every other day but I have heard of guys using it more frequently such as on vacation. They instruct you to start with a lower dose (10 or 20 units). If you don't get the desired result increase the next time by 5 or 10 units until you have a firm erection. As I said, the issue is it staying hard for several hours. If it stays hard for more than 4 hours they instruct you what to do. I've read that this is rarely an issue and is usually because someone takes Viagra at the same time. No need to do that.
The injection really isn't a big deal. Very small needle like used by diabetics. Goes in maybe a third of an inch. Works within 10 minutes or so. The first time I used it it was like magic because I hadn't had a real erection for over a year since surgery.
