Do you know about Mayo Clinic's free Concierge Services? They can help make recommendations suited to your requirements. It's worth contacting them if you haven't already.

Have you asked anyone on transplant team about recommendation? Is there a social worker on your care team? If so, they may have recommendations from previous transplant patients.
