Incontinence Improvement with Pelvic PT?

Its only been since December 7 that I've been in PT and recently began tracking using the 24 hour pad test. Since February 11 the average is 444/24 hours. The 2 day moving average is 418/24 hours.

This looks like I'm far behind the curve of those who eventually made significant improvement.

I'm interested in other's comments about their rate of improvement over the initial months following RP.

I do my daily kegels, and other exercises from PT. No caffeine or alcohol. 73 years old. No problem prior to RP.

Once my catheter was out, my urologist recommended trying to urinate on schedule. In other words, start by urinating once an hour, whether you need to or not. Once you’ve mastered that, gradually increase the time. I believe 2 hours is the goal. Remember that while the catheter was in, the bladder could empty freely. You need to retrain it to work as it was meant to. A nurse also told me that it’s better to drink normally, that a fuller bladder is easier to control. Since maintaining continence at night is a problem, I would still limit intake after 8 p.m.

Thanks so much this makes perfect sense.
I need to retrain my pelvic muscles and my bladder
One thing that I have learned is that certain days when I babysit or have a change in schedule my bladder training gets interrupted as does my progress. Going for a walk ( which I really need to do) or getting up and down with grand kids, causes significant stress incontinence (emptying my bladder whether I want to or not) when I go for a walk or babysit for several hours.
I can train my bladder best if I can more or less sit around which I guess is unrealistic/ I guess this is just part of real life getting in the way. Anyone have any little tips about this? How leakage caused by moving around interferes with scheduled bladder retraining?

A few ideas that worked for me -- find a pelvic floor physical therapist who treats men, not all do. Next lose weight if thats an issue. Avoid bladder irritants like caffeine, alcohol, citrus, etc. There's a list on line if you need it.

Don't drink less fluids, the bladder is a muscle that needs to be stretched like any other muscle.

Weigh your pads if you are not already to see the change and what activities trigger leakage.

Learn to brace yourself so you don't leak on getting up and down and going up and down stairs.

Be patient at our age (almost 70) it takes time.

I think I have a good therapist who is not only familiar with men but familiar with prostratectomy.
I used to drink several cups of coffee but am now just drinking one small cup early in the morning. I do not drink, smoke or anything else. Each time I stand, sit, go up and down stairs even walk unless I walk very slowly , I leak and throw off bladder training.. This is till relatively new but being 71 is also a likely issue

I can't say this is a great recommendation because I'm not a doctor or a therapist, just a former owner of a prostate that lost it two years ago and went through most of the problems you have read about. One thing I have loved having is a condom catheter. I use it instead of a penis clamp because it is far more comfortable. Perhaps until you develop your pelvic floor you can sleep with a condom catheter on and make it through the night. You can attach it to a bag or just clip it shut (with a paper clamp) to empty when you feel the urge throughout the night. My doctor's office nurse got me in touch with a supplier and Medicare pays for the monthly supply.

Interesting ! My penis is now smaller than a cocktail wiener and never gets longer. A condom wouldn’t stay on .

I may keep the condom catheter in mind if this goes on for months and months. I guess at this point, I am reading that a big majority of men need three months at least ( and some say this applies to those between 40 and 60 not in their 70s) I hate incontinence, I hate not getting good sleep but I guess I will continue with bladder re=training and kegels and now no coffee to just see if I can get back to what was normal

The company has a sizing system and sends samples for you to choose the best from. They’ll have one that fits.

You can learn to do Kegels while sitting, standing or laying down. My understanding is we need to learn to isolate them from ab muscles. Full bladder or empty bladder too. I am 26 mos post removal and undergoing radiation which requires a full bladder. Kegels and controlled breathing while on the table. Many good sites on the web for pelvic floor strengthening for men.

Yes it’s a life changer. One thing I do that works is at night after dinner , after kegels I put on a depends sure fit diaper, all night very little dribble. And maybe once go to the bathroom.
