UTI Urine tests and 2 blood tests indicate CKD - so confused
Hey everyone - I've been reading the threads and all of you are SO supportive and have a complete wealth of valuable real life knowledge. It's refreshing.
Im 56 F and I got my 1st UTI last week. It was scary and literally when I did the urine to be tested all I could see was blood. I have anxiety already so you could imagine my flip out!
I've been at 61/60 egfr for years. It's part of my normal bloodwork. Foc doesn't flinch - my creatinine and BUN always normal. I am always aware because I lost my brother 6 years ago at age 60 - Diabetic - lots of bloackage in the heart - no excercise - ate like crap - lost 1 leg below the knee and eventually was on dialysis for 4 years. Then he got something called calciphylaxis which basically according to Dr Google is a rare and severe skin condition that occurs when calcium deposits build up in the small blood vessels in the skin. This buildup blocks blood flow, leading to skin damage and ulcers. Usually happens with patients on dialysis. So I have ALWAYS stayed at PRE-DIABETIC , sugars good , A1C good etc -- Cholesterol good except for elevated tris. I have been on Zepbound for a year and dropped 50lbs so I am almost under 200lbs - 5'6". So I do all this - eat well - low fat - low sugar - I get a UTI for the 1st time and went to a doc in the box ( urgent care) - they did the Urine test and a million blood tests my reg doc does.
LONG and the short - my Microalbumin/Creatinine Ratio came back sickly crazy at 1096 and supposedly it should be no more than 29 ! So that's from the Urine - then the metabolic panel came back - all normal except my eGFR is 44! and my blood creatinine is 1.41 - BUN is normal - plus the CBC came back with all the elevated tests you would expect from a UTI
So send my doc the results - says come in and chat Monday - it's NOT an emergency - OK but hello? Egfr is 44 the ratio is 1096 and I have a slightly elevated Creatinine.
OK so firing off questions -
I don't drink a lot of water - the night before I got 4 charlie horses in my legs - totally dehydrated I'm certain AND I got this UTI - are these results accurate? Wouldn't blood suggest it was slightly affecting my kidneys so the tests would be skewed?

No mention of kidney issues last year when I had all my reg blood work done. I'm freaking out for obvious reasons with losing my brother but I'm also a little angry that this doc in the box didn't tell me that the eGFR could be messed up due to this UTI???
Any experiences you've had with a UTI and these numbers or dehydration - please toss them over! I am flipping out!

Blood in the urine, UTI and antibiotics raise creatnine. Unfortunately, so can Zepbound. Zepbound can increase
the excretion of glucose in your urine providing a favorable environment for bacterial infection.
I'd suggest that you stop the injections until you talk to your Zepbound provider. Zephound can cause kidney damage by causing dehydration.
You could probably stay on the medication as long as you stay hydrated. I don't think your test results will be accurately reflected until you have vanquished the infection and stopped the antibiotic. I hope your numbers will be back to normal.
I understand being anxious after getting tests results that surprise you. I had something in mine once when I went to Urgent care for a suspected UTI. I went crazy with worry. I didn’t have all the tests you had, but it tested for blood. I was convinced of the worst, but later I saw my primary and got another test and all was fine. I didn’t have kidney disease as I feared. (I have longtime type 1 diabetes .) I know that egfr can fluctuate. So, I’d discuss things with your doctor before getting too upset. It might not be what you think.
Thanks for this! I had no idea about Zepbound - probably b/c I never knew there was anything going on with my kidneys! Always maintained ion the 60s for years. I am going to see my doc Monday and I know he will say let's repeat all these - that eGFR# was like being hit with a cast iron skillet! THANKS for your reply!
I am SO anxious around medical stuff - the things is everyone has MY CHART or a form of it so 99% of the time I get the results before the doc and have already diagnosed the worst!

Drink water - this is critical to keep the toxins in your system flushed out. My understanding is the eGFR needs to remain low over time to really be of concern to your doctor. I say this knowing my mother died 60 years ago (I was 23) with kidney failure - before the days of dialysis. I too watch this portion of my systems for any signs of problems. Recently my eGFR has been low (for 2 years) and I am currently being tested by a Nephrologist to determine if I have a problem and if so what, if anything, can be done. I also have a floating kidney - a kidney that is not tacked up where it belongs, so it floats in my body, causes constant pain, but is also doing 60% of the overall work. This kidney can get pinched and be a major issue - I am 81 and so far it has not caused any major issues, just another item to keep an eye on. Hang in there and try to let the doctors run their tests and make their decisions before getting too concerned. Hard advice for me to follow - so I totally understand.
@swbwnwsictis1 welcome to Mayo Clinic Connect. I’m sorry for the loss of your brother. Though you and your brother are related, his health journey does not have to be your journey. It sounds like you are doing a good job of researching his issues and doing the best you can to prevent the same problems. I’d like to reiterate some common themes you will see in these discussions.
One set of lab results is not that useful. It is the trend of results over time that gives more information. For example lots of factors affect the eGFR. The trend is more important.
A UTI doesn’t necessarily mean that you have kidney problems. A UTI can happen for lots of reasons.
All of us members are not your medical providers. We shouldn’t be diagnosing or prescribing. We tell our stories to encourage and support each other.

Absolutely understand no one is a medical professional on the forum. I think I was more concerned about the eGFR and a slight elevation in Creatinine caused by a UTI - Found out the eGFR is calculated by the Microalbumin/Creatinine ratio - I reread my labs and the Urine Culture came back as "contaminated" no I'm thinking that ratio is off way with this new finding and the UTI so I am hoping the eGFR is off also. Seeing doc tomorrow to ask all my questions,
I came to the forum to get some advice around what other people have experienced. Like the one thing you mentioned about trend being used to diagnose where I am at.
The people who responded have been extremely helpful in calming the nerves when it comes to initial bloodwork and urinalysis due to my UTI.
@jabrown0407 first, I am so sorry that your mother went through this at an early age where dialysis wasn't even an option. You know too well what to look out for and I can imagine the "floating kidney" makes things a little more tricky!
