Carotid ultrasound for tinnitus. Results normal part of aging?

Posted by sandrajane @sandrajane, Jan 16 3:12pm

This US was ordered by my ENT for tinnitus. . He reviewed
it and texted me today that there are no alarming issues noted. I did reply that I was concerned about the stenosis and the artheromateous disease,.
Should I be concerned that he is not concerned or is this a normal part of aging?
Narrative & Impression
General: A bilateral carotid duplex was performed.
Previous: No previous study.
Right: RIGHT: Doppler flow velocity represents ICA stenosis of 1-39%. Heterogeneous
plaque noted.
Proximal ICA PSV/EDV (highest) = 112/26.3cm/s
ICA/CCA ratio = 0.9
Antegrade vertebral artery flow noted.
Left: LEFT: Doppler flow velocity represents ICA stenosis of 1-39%. Heterogeneous
calcified plaque noted.
Mid ICA PSV/EDV (highest) = 106/31.4cm/s
ICA/CCA ratio = 1.0
Antegrade vertebral artery flow noted.
Conclusions
1. RIGHT ICA: 1-39% ICA stenosis - (Mild atheromatous disease).
2. LEFT ICA: 1-39% ICA stenosis - (Mild atheromatous disease).
3. Antegrade vertebral artery flow noted bilaterally.

I feel inadequate in answering your questions, but since no one has commented and you sound anxious, I will share my experience. I recently had a carotid US. My numbers are nearly identical to yours. I am not concerned nor is my cardiologist. The US says you have a mild, slight amount of plaque and no significant stenosis in either carotid bifurcation. You have adequate cerebral blood supply and oxygenation.
How and why do you have a slight amount of plaque in your carotid artery?
You didn't mention your age, if you've had a CAC scan, or if you've had a comprehensive lipid panel including APOB and Ap(a).
If no one else replies, ask chat GPT to interpret your results like a functional medical doctor. I did this for one of my blood tests and I did a good job explaining so I could understand.

Thank you for replying. Age 66.
Last blood work was 2 years. Total cholesterol 253. LDL 164. Hdl 75 tri 75. All my ratios come out great.
Don’t see APOB & apA. I will ask for them next month.
I have not had the scan either. Will request that too.
Thank you again.

I'm not sure about your ratios being great. Your LDL is high. I'm glad you are taking the initiative and will do more proactive heart tests. The scan and blood tests are easy, noninvasive tests.

My ratios are great. I did them all.
LDL/hdl= 2.216. Ideal
Total/hdl= 3.42. Good
Tri/hdl=1.0 ideal
Mesa risk score is below 5
I may not do any more tests. I’ve been reading that everyone over age 60 has some plaque buildup in their arteries. I’m not going to let my doctor make me take terrible side effect drugs for normal aging.
