Usage of Immunotherapy , HIFU , Cryoabalation , NanoKnife , TULSA PRO.
Apparently, these treatments really only work properly with somebody who has a Gleason of 3+4 or less. I have been recommending looking into them for people that have higher Gleason scores, It appears I was wrong about that and they should not need to investigate those techniques because they aren’t going to work well with those higher Gleason cases.
A 3+4 with intraductal or cribriform would also not be looking for those treatments.
I would love to hear from somebody in here that has had one of those treatments with a high Gleason score, and how successful it was.

I had Tulsa Pro for Gleason 7 (4+3) at Mayo Rochester (I am part of a clinical trial). At 3 months my PSA was down to 0.68 from 8.6. Time will tell how successful. I go in for my 6 month PSA and MRI on Jan 21. I have had no side effects.
Did you have intraductal, cribriform or SVI? Those factors are aggressive and affect procedure choices.
Results sounds good, I suspect it’s like radiation. You will get lower and lower as time goes on.
I had a 7 (4+3) and underwent TULSA at the end of November. My Dr. said I was a good candidate for the treatment. The MRI showed that all the cancer was gone after treatment. I go back in March for follow up.

I had not found this discussion board before I had my procedure. If I did, I would’ve had a lot more questions to ask the doctors. However, the team at Mayo looked at all my data and decided Tulsa pro was a great fit for me. Here is some verbiage from my pathology report: Cribriform glands are present. Peri-neural invasion is present.

My Tulsa took about 3 hours. I had the catheter in only 2 days. I think my catheter time was short because they only ablated 30% of my 45 cc prostate. Catheter was annoying but not a big deal. I went for a 2 mile walk the day after Tulsa and carried my catheter bag in a shopping bag. Zero pain from the Tulsa procedure for me.
Your prostate at 45cc is at the lower end of the spectrum . Mine is on the borderline of LARGE , at 80cc . Larger prostates present a greater challenge with TULSA PRO .
Here is my Tulsa Pro story (with others mixed in). My Prostate was over 100cc, but it is still possible to do Tulsa. What they want is the temperature up where the lesion is long enough to really ablate all cancer. Unless you are much bigger than my prostate was, it is still possible. I am one year out now, no cancer seen on recent MRI, so very pleased. I do have to deal with the BPH I am getting, looking at things for that including what is on the "Game changers" video on netflix and paid youtube about not getting things like these from meat, basically by skipping meats: Heterocyclic amines, TMAO (Trimethylamine N-Oxide) , Heme iron, Neu5gc, AGEs
But at the same time most people unless your cancer has spread outside prostate, or your lesion is located in the outer edge of a 200 cc prostate (any huge size) or something you will qualify if you want to get it.

Now keep in mind some places are doing Tulsa on only 3+4 because they are doing a study for papers where that is the mandate of the study and for making the paper quicker to publish. So that is just for the purposes of making a quick publication. It has nothing to do with excluding people who can get Tulsa as it will burn any grade tumor up. So if you hear that go to another Tulsa place.
