Alpelisib (Piqray): for anyone starting this medication
I’m posting this update to my original post re: Piqray so that maybe it will be helpful to women who are starting this med. I was not able to find posts related to this treatment.
I”ve been on Piqray ( Apelisib) for 1 month in combination with monthly Faslodex injections after having been on Ribociclib/ Faslodex for 15 months with new liver mets. This drug is specific for breast cancer patients with a PIK3 mutation of which I have. The most common side effects are hyperglycemia and diarrhea. Prior to starting this med I tested my fasting glucose via glucometer daily for a week to establish a baseline, then daily for the first two weeks once I began Piqray. My fasting blood glucose continues to be within normal limits and I test a few times a week. I’m wondering if the incidence of hyperglycemia increases over time. The GI symptoms have been manageable with Immodium as needed.

Hello @hikerlou I am glad to hear that the side effects are manageable. I am very enthusiastic for the new drugs that will target gene mutations instead of just hitting all the fast growing cells in our bodies.
Do you have a plan for timelines of imaging to check up on those liver Mets?
Yes. I have CT of chest and abd, and MRI of abd scheduled for January. The CA 27.29 antigen has been a good indicator for me and will continue following that as well.

Please see my first post above re Piqray. Like your Mom I’ve been on the Ribociclib/ Faslodex combo. It worked for 15 months. I’ve been on Piqray for almost 2 months. No rebated blood sugars. YET. I’ve had a few bouts of diarrhea but that seems to have stabilized. I have noticed mouth sores and a metallic taste in mouth. Please let me know how your mom is doing on this med. There aren’t a lot of women on it.

Thanks you for checking in. The injection was fine. Other than a little tenderness in the area of the injection I feel good. I have my second one on the 22nd. My doctor delayed the start of PIQRAY as my glucose was high. The plan is to start me on a 7 day med and then start PIQRAY. I’m having a spine and skull MRI on Saturday and they want to see the results of that. I fell like I need an Excel spreadsheet to keep track of it all. How are you doing? Did you start the PIQRAY? How are the side effects? Take care.
