who is a good candidate for Intermittent ADT?

Who is a good candidate to take a holiday from ORGOVYX until psa start rising and then go back to taking ORGOVYX again?
Is it only applicable for non-aggressive cancers or High Gleason score or metastasized cancer?

I'm 60 now, with over 3 years at stage 4 and no progression. The new miracle drugs like -lutamides (and whatever comes next) might be able to keep me in remission for years or decades, but that also possibly means decades on ADT+ARSI.

So far so good — I'm getting stronger every month thanks to weight training and a lot of walking (I can't run since my spinal compression, but at least I'm not in a wheelchair any more), I'm trying hard to manage my weight, I'm taking calcium supplements, etc. etc. — but if I'm lucky enough to keep the cancer at bay, what's my body going to be like after 10 years on ADT+ARSI? 15? 20?

People with stage 4 prostate cancer didn't used to live very long — Canadian politician Jack Layton was diagnosed with advanced prostate cancer in 2010 and dead before the end of 2011, for example, and the *best* case survival was only about 5 years before the new drugs and treatment strategies came in just before the pandemic — so I think the medical community really doesn't know what we're up against long term. We're the guinea pigs. But again, better a live guinea pig than the alternative.

It's become an ongoing question on my "Questions to Ask My Oncologist" I just entered year four after diagnosis and my oncologist always answers respectfully "Hmmm, not right now." But, I keep asking anyway.

Forgot to mention, over the last six months, Total Testosterone went from 3 to 82.

Based on your test results and my knowing many people doing what you are doing in the weekly advanced prostate cancer sessions I attend, it sure seems like a safe bet that you could stop ADT. Just make sure to get PSA tests now and then.

Yes, "non-mestastic" was probably too broad a brush. Let's say "non-metastatic or local spread". It's advanced prostate cancer with distant metastases where there's not evidence yet to support ADT holidays (as far as I know, and that could change tomorrow if a new study publishes its results).

Gleason of 4 +3 +7; no metastases. Had Lupron D injections every 3 months for 1 year. PSA immediately dropped to undetectable with testosterone level of 10. 15 months from onset of treatment and last injection May 13,
PSA remains undetectable and testosterone is still 10! Side effects have dissipated and Urologist is willing to do active surveillance with next blood draw in 5 months.

You don’t sound like a subject for active surveillance. 4+3 is somewhat aggressive. Yes, Lupron can stop it from growing, but you’re likely to go castrate resistant because of your not having any treatment. Did you get a decipher score? That’s a good way to find out your chance of future problems..

You need to speak to more than a urologist, they are surgeons and not much more for prostate cancer. You should go to a center of excellence and get a full workup. You really are taking a chance relying on that urologist. They are not prostate cancer specialists. I know a lot of people that really regret listening to only their urologist when they first had prostate cancer.

Did you ever have a PSMA pet scan? You really need to do that to see if your cancer has spread, but you can’t do it while on Lupron with such as low PSA because it really can’t detect much of anything.

To get more information, you should sign up with Ancan.org and attend their advanced prostate cancer meeting on Tuesday. They have weekly two hour online meetings. The people there have 15 years experience handling prostate cancer treatment. There’s always at least three doctors at the meetings. You need some expert advice and they can give that to you. You probably would be more suitable for their low/intermediate sessions, but you could attend the advanced one to get help right away.
