Doctor is concerned about Kidney GFR numbers: Should I be?

My Dr. is showing concern about my GFR numbers, 1 year ago it was 122, half year 112, Aug.2024 90 and Sept 79. Should I be concerned?

I was lucky enough to receive a kidney transplant recently and have 5 nephrologists on my transplant team. Without exception, they say they don’t care about eGFR (because so many causes for radical change) and look only at creatinine and BUN tests. I was surprised too!
Also, when first diagnosed with CKD was taken off ALL OTC medication except acetaminophen.
That is interesting. I was in the ED twice in the last 2 years for UTI, Covid, and sepsis. Both ED DRs were only concerned with creatinine #s, but my nephrologist always looks at eGFR. Aren't they related? A calculation of both? My BUN is around 30 and creatinine 2.2, eGFR 23 (but changes from test to test). Are these the numbers I should be concerned about? I drink a lot of water, 60+ oz a day. But my disease has me in a state of dehydration due to tubules not retaining water. My urine is always very light, like a light lemonade.

Yes, one measures the other, but as you see from your eGFR it changes from test to test. Creatinine and BUN are what they care about, with me anyway. Yours are both a bit high.
I drink 6 liters minimum of water daily, (about 114ounces) and did that even before my transplant, although it is really hard to get down. Your urine is excellent. (Did you ever imagine having a conversation like this lol)
My creatinine in Nov. 2017 was 1.34. Oct. 2024 it is 2.39. An increase of 1.05 in 7 years has me looking at dialysis or a transplant, in a few years, maybe sooner. Do you mind sharing your creatinine level before your kidney transplant? I know everyone is different. I do not feel symptoms. I can walk 2+ miles/day and am awake and feel normal. Everyone is unique as when symptoms begin.
My creatinine levels ranged between 3 and 5.4. My energy level was very low but i played a little golf and pool aerobics.
Are you looking at eGFR or GFR.

siosal, I couldn't even get results from the large clinical trial. You might try contacting one of the authors. I've gotten access to journal articles that way in the past. Your doctor or the patient's doctor probably has access through institutional connections. Asking them for a copy would draw their attention to a study that they may not have seen yet.
I have also paid for access to a single article in cases involving my parents, usually it is around 30$ for a month's access to a single journal article.
