What should I know before starting Evenity?

EVENITY
Like many of you, I have started from shock, to fearfulness to being
apprehensive what medication to take for severe osteo.
My numbers are L -5.2 hips -3.6 and -4.0
I hoped to be able to improve the numbers with diet and exercise.
Eventually I realized that it might be possible with osteopenia
but probably not with severe osteoporosis. Dr Beck
Onero program is not available in Vancouver, yet.
I am trying to learn hip hinge and squats
eventually I will find a a physical therapist/kinesiologist
with osteoporosis experience.
I do the stairs as often as I can, increasing how many times:
until hopefully add ankle weights.
My threadmill speed is 2.2mph..the goal is to increase every few weeks:
method: increase, count 24 seconds - go back to starting baseline – several times:
the goal is 2.5mph which is maintenance: and the larger goal is 3.1mph
which is osteogenic.
Diet: aside from the calcium and and Vit D: am looking into natural
sources of Vit K (Natto is not something I could consume regularly).
and have 5-6 prunes daily.
So…have finally made the decision to go with Evenity, if approved.
Have not used anything in the past: anabolic or anti resorptive.
Thank you for ladies from Canada for mentioning CTX. It is available
in Vancouver (sorry I didn’t mark down names) P1NP and TBS is not available.
Neither is REMS. I got a CTX – which we pay out of pocket here, so is
Vit D out of pocket unless it’s requested by a specialist. And TBS is
not available either. My CTX was in the 500.

sci-hub can be wonderful for older papers. At some point there was a crack down on their efforts to make these scientific papers available to everyone. Not sure what year that was but something like 2020. Before the crack down they had lots of journal papers that most of us could not access or afford to pay $30-$50 to read.
I remember feeling this way when I started on birth control and felt like a guinea pig ...
As I do now with osteoporosis meds.
My bone health specialist has recommended I start Evenity as I have osteoporosis & have had a spontaneous spine fracture.
I had bariatric surgery 18 years ago & suffer from malabsorption & despite taking 1,800 mg of liquid calcium citrate concurrently with 15,000 IU of liquid Vitamin D3 (split into 4 doses per day) & we can’t get my calcium levels up in the normal range. Evenity warnings specifically state not to administer to patients with low calcium levels so we keep trying by increasing calcium citrate & D3. I do not want to start Evenity until levels are normal. Has anyone else experienced this problem or can offer any advice? Thanks!
Wow that is a high dose of Vitamin D3 @manco! What is your current D3 level?
Maybe you could do Tymlos or Forteo since they raise calcium levels! At least for a few hours. Not sure what level is needed for those to work. What does your doctor say?

@hopefullibrarian Hey, glad you liked that. I was really excited when I first found Sci Hub. I had been disappointed for years being unable to see papers and research I would see referenced but could not look at.
Note that the url will change at times. I'm not sure what goes on but I envision a network of computer free spirits who think research should be available to all - and then organizations and governments who don't like that work together to shut them down. And then they spring up somewhere else. Always called sci hub .... it seems. As for as I know they are now limited in that they can't add new work. That said I don't know the details so I could have some of this wrong.
I thought it was very high, too. Last Vit D-Hydroxy on 8/5/24 was 35 ng/mL (normal range), but calcium was 7.8 mg/dL on 8/5/24 (low). Last calcium on 10/4/24 was a bit higher at 8.1 mg/dL, so calcium level does seem to be improving. I had been taking 1,500 mg of liquid calcium citrate when it was 7.8, now with the increased 1,800 mg of calcium it’s up to 8.1 (getting closer).
Doctors recommend Evenity for me. I have an upcoming appointment & will ask them again about Tymlos & Forteo.

You make me question whether or not it’s true that we can’t maintain bones naturally.
You suggest that we can maintain bones naturally. How do we do that?

Personally I believe we will continue to lose bone after menopause but we can slow that loss with diet, exercise, supplements and generally reducing inflammation. If we are not in danger of fracture and have scores that are in a better range. Keith McCormick first wrote "A Holistic Approach to Osteoporosis," and his more recent book "Great Bones" is encyclopedic. Margaret Martin at melioguide.com is helpful on exercise. Talk to your doctor but at -1.5 or -1.0 I would not medicate but monitor closely. I believe many docs are not medicating osteopenia these days.
@mancos one other thing to think about: I read that Tymlos "eats D3" and I kept mine stable at 50 or so by upping my intake to 3,000 units/day. When I stopped Tymlos my level started to rise so I backed down to 1,000-2,000 units depending on the season. I don't wear sun screen!
I cannot believe you take 15,000 units and your level is 35. Do you take a liquid, or gummy, or pill?
