Pain in the butt - Bilateral Hamstring conjoint tendinosis
I've had sit bone pain for over a year now, and finally got an ultrasound that showed tendonitis on upper hamstrings and some calcification of the tendons as well as thickening of the bursa sacs and partial enthesial tearing at the ischial tuberosity. This was after months of myofascial release therapy and exercises from a pelvic floor specialist that didn't help. Last month, I had cortisone shots around both ischial tuberosity joints and went to a new PT who did some scraping on that area with a graston tool as well as stretching and strengthening exercises. The pain is now worse, so I'm going back to the doctor who injected the cortisone next week. I'm wondering if anyone has had successful treatment for this condition. I've seen posts about enzymes and supplements like Turmeric that help, but I'm on a blood thinner so most of these would pose a bleeding risk. Any suggestions are appreciated.

I see similiar questions on here regarding the upper right buttock pain but I believe mine may have enough differences to warrant another question. 10 years ago I took off my house framing toolbelt and hung it up for a supervisor position believing it would be better for my body. A year later I was still putting it on occasionally but for the most part I was supervisor only. The da my pain first struck I put my belt on in the morning and walked about 200 feet. The pain hit hard and I had to take the belt off and sit down. When I got back up I could barely stand or walk. The pain was excruciating in my upper right buttock. I saw the doctor and went to physio and the pain subsided but never went away. Today the pain is still with me and it's really affecting my life now. I can sit or lay all day long... I can run, or walk with purpose for any length of time, but I can't stop and stand for any length of time. Activities that require me to stand for longer than a couple minutes like my job as a supervisor, standing in lines, cooking, standing at a sink to wash something or groom, working on a car, standing and chatting, shopping.... they all cause pain. Lately my right knee has been bothering me as well, usually after the butt pain starts to flare up. It may be a separate issue but I thought I would mention it. I should also say it's not radiating. I have isolated pain in the butt, and the knee. I have done extensive research but can't find any one thing that it could be without a doubt. I guess I'm leaning towards sciatica or compression fracture. Can anyone say they have similiar symptoms? I recently had another x-ray and I am waiting to speak with my doctor for results. Unfortunately my confidence is not high for a successful diagnosis.. I figure if they didn't find it 10 years ago they likely won't now either. Can anyone possibly provide some more insight?

I too have that problem. I have not had the tests that you’ve had but cannot sit for long without, what I call, sitting on a stone. My Docs , massage therapist and PT feel a thickness at the top of my hamstring but so far nothing has helped. I do have many other pain problems so this has been secondary but when other stuff improves I always come back to the pain in my butt that no one has an answer for! I have pain in my butt also (gluteal pain) that is a burning ache like pain; but unlike you, it only hurts when I walk. Although I have some torn muscles, several docs I have seen are convinced it is due to my spinal stenosis. Interestingly I had major spinal surgery a year ago to address the stenosis and although it cured my sciatic pain, the burning butt pain continues (which I had before the surgery, so it' obviously not from the surgery). No one knows what it is or what to do. I have been to Pain Management, a Neurologist, an Orthopedic, PT, Sports Medicine and a Vascular doc ... no one can figure it out. Someone else on this site had something similar and felt it could be neurogenic claudication which I am no researching. If anyone has something similar I would love to hear from them.
OMG! I’m not losing my mind!!! Over the past year and a half I have had pain like no other. It feels like I’ve been cut in half with a chainsaw and tazed all at the same time. I am 68 and have bilateral labral tears. I’m trying to lose weight for a hip replacement. I also have scoliosis with a herniated disc in part of the curve. No surgery due to the location. Just spinal steroids. I too have sitting on a sharp rock feeling which has gotten worse recently with the addition of the horrible hamstring pain. I figured the pain was related to the herniated disc. Early on in this adventure, a pelvic X-ray showed tendinitis in my buttocks, but then the labral tear was found. So with the scoliosis tipping my pelvis, the herniated disc, the labral tears, why wouldn’t the hamstrings get pissed off! And Tylenol or Motrin is causing my hands to ache. ☹️ back to tramadol and Robaxin.
I have the same issue, all on my left side. My pain is right in the center of my butt but also in the front of my hip. I have hamstring pain also. My knee is the worst pain and very swollen. An orthopedic surgeon is looking into both my hip and knee. I’m waiting on the results of an MRI aid my knee then we are injecting a numbing medication into my hip to see if it relieves pain in my hip and or knee. The buttock pain started after the hip and knee pain. I also have lupus, RA and Fibromyalgia.
Yep! It sucks. BTW a I also have labrum tears - those are new as of Nov. I do have back issues too! All moderate so no one really pays attention . I am seeing a new Physiatrist whose specialty is “ spine” Maybe???
I have had hamstring and gluteal pain which causes pain when sitting for almost 3 years. I recently had a MRI of my pelvis which showed full thickness hamstring tear and probable gluteus minimus tendon tear. I was advised to have a steroid injection into the ischial bursa. If that doesn’t help the next steps are PRP injection or shock wave treatment. I feel I was fortunate to have the pelvis MRI because at least now I know what is causing my pain when sitting.
“ I feel your pain, literally”! LOL! I have had the same pain going on about six months. I have pinched nerves in my lumbar and have had steroid injections and Radiofrequency Ablations for that which didn’t work. I didn’t have the sit pain while sitting until after the ablations but I had gone to a chiropractor and he taught me some deep stretching exercises and I think the deep stretching may have torn something . We did do a repeat MRI after returning to the Pain Dr., but I haven’t had my follow up and I think the not so bright Dr just did the MRI of the lumber again because the report which I can see didn’t say anything about my ischial tuberosities. I did tell her I have pain sitting. MRI did say I have severe facet arthropathy/ arthritis. I see the pain Dr next week. All said you mentioned doing a steroid injection in that area, so did my Dr but I have had 4 in the last six months and my HgA1C and triglycerides went up. Will your insurance pay for RPR????? Most don’t. I do different supplements for inflammation, I can’t take Pregabalin because I have severe colon inertia and the Celebrex that I asked for irritated my stomach so I stopped. I’ve helps! I’m tempted to spend thousands out of pocket which is what it cost for RPR so I can sit . I work a desk job so it’s hard and we also make a 750 mile car trips to see my daughter every 4 months. It’s a miserable life not being able to sit. I stand a lot so that hurts my back more. I have every kind of cushion on the market and have made home made ones with wholes to take the pressure off but they work about 50%. I’ll be stuck with another MRI bill too. Drs today don’t impress me. That is why this site is good because your Dr may be better than mine and are better diagnostician's . I don’t have a problem telling a Dr what I need and of my health concerns. How do you think you tore your hamstring?

I have cervical/lumbar spinal stenosis and degenerative disc disease. I had pain in my hips/buttocks that went down into my legs/feet at times. I had a MRI of my pelvis/hips and it showed I had bilateral gluteal tendinopathy and bilateral partial hamstring tears! I have no idea how and when this happened! I had lumbar spine surgery on 8/2 (decompression/fusion L3-L5) which was very painful but my hope is it will relieve some of the neurogenic claudication symptoms in my hips/buttocks/legs/feet. I wonder if my pain/weakness/numbness in that area caused me to tear tendons/muscles.
