What is the recommended treatment after 6 Pluvicto doses?

I will have had 6 Pluvicto doses next month. PSA has decreased some. SE have been much easier to tolerate than chemotherapy. What is the recommended maintenance treatment after receiving Pluvicto?

Can Pluvicto be repeated?
Has it been in EU where it was approved longer ago?

Xofigo is a radiopharmaceutical similar to Pluvicto but is only effective against cancer in the bone and will not treat cancer in soft tissue or lymph nodes like Pluvicto does. I received four doses of Xofigo After which my doc pulled the plug as it was not doing me a whole lot of good so he started me on Docetaxol x 6 doses so that I could qualify to get Pluvicto (which currently requires treatment with chemotherapy first).

Time is needed for bone marrow to recover after chemo and or Pluvicto. With regard to other treatments - I understand some doctors may recommend combing Docetaxel with Caroplatin instead of Pluvicto or as an alternative treatment. It also would depend on whether one is chemo healthy. What ever happened to cyber knife? Wouldn't that be another alternative.

I read that Radium 223 (Xofigo) works well for PC in the bones and that it should be used before receiving Pluvicto. It seems to me that many doctors want to use Pluvicto because it is so new. It also seems that there is a movement to get away from Chemo - maybe because some men are not chemo healthy or it is too debilitating.

Lastly, there is a clinical trial going on in South Carolina called the TAMARACK.

The Tamarack clinical trial uses a new generation of therapy, MGC018, which targets advanced prostate cancer. MGC018 is antibody-drug conjugate (ADC), which carries a chemotherapeutic drug and delivers the drug prostate cancer cells. This targeted approach to cancer treatment allows for higher drug concentrations with less damage to healthy tissue. The primary objective of this clinical trial is to see how MGC018 performs in patient with mCRPC

I was just diagnosed with metastic PC in January. I'm on Abiraterone daily and my PSA dropped from 359 to 69 in 3 weeks. I got a Lupron shot 2 weeks ago. Waiting for my next PSA test to see how I'm doing. Pluvicto was something I read about in my panic to gather info. Did you good through similar treatments as I did before starting Pluvicto? How long ago were you diagnosed? How often do you get the Pluvicto?

@digger My husband's first treatment with Pluvicto was last week. Upon advice of the medical team, we used three days not sleeping in same bed or close contact and 7 days for no contact with children. Another tip -- RNs with the nuclear medicine team administering the treatment emphasized the importance of taking anti-nausea meds immediately after treatment to prevent symptoms (nausea, vomiting). He did that. His only side effect has been fatigue. Good luck on your treatment!

I just finished my 2nd round of Pluvicto, I am feeling very fatigue, lower back pain, exhausted, shortness of breath, almost non functioning, non stop urination, and confused at times otherwise everything is ok.

First round for me was exactly the same. This second round has been a lot tougher.

After a significant delay - 4 months- I will resume Pluvicto, with careful monitoring of side effects and PSA. Can find no related data.

Hello Dave,

I have had 5 Pluvicto treatments so far. Number six coming up in a few weeks.

I bought a dosimeter so I could check on my actual radiation reading instead of just blindly following the official guidelines of staying three feet away from people for three days.

Based on my actual radiation reading history, my personal protocol is to isolate myself completely for one week, i.e. no physical contact, no sleeping with the wife; then for the second week allow brief physical contact - a hug or a handshake ok but no prolonged contact such as cuddling or sleeping together. After two weeks my radiation level is back to "normal" and full contact and sleeping together is ok.

If you are a person like me who wants direct knowledge and wants to make your own decisions concerning your own health, treatments, and how much radiation exposure to your loved ones and caregivers that you personally are ok with, I urge you to do what I did - purchase a dosimeter (about $100 on a Amazon), take daily readings (one at arms length and one touching your body) a come to your own decisions.

Russ is on his 3 rd this week at the Mayo Clinic in Rochester Mn ! He does quite well after ! Dr Childs and his team are awesome! They all tell us that u are fine being together just no intimate or kissing for 3 days ! If you have bladder issues he would wear a depend ! He doesn’t have that issue !🙏😊 best of luck on this fight
