ANA Positive - Mitotic Spindle Fibers and Nuclear Speckled
Hello,
I've been getting positive ANA results since 2014 - in 2014 they were 1:1280 speckled, in 2016 they were 1:640 speckled, and in 2022 they were 1:1280 nuclear speckled (AC-2,4,5,29) and mitotic spindle fibers (AC-25). In 2014 and 2016 I had more antibody testing done and everything was fine. I'm set to get more testing done soon.
Just wondering if anyone has had a mitotic ANA pattern, including spindle fibers or NuMa, etc. From what I've read the mitotic pattern isn't yet known to be linked with any specific condition, so I'm interested in hearing from anyone who has had it.

Good morning, Lynne. How did you get your lab to look beyond the automated processing of your ANA pattern? Mass General Brigham never seems to put human eyes on interpreting the ANA pattern. Knowing your pattern is mitotic is more than patients normally find out.
Did you chant a magical incantation? Please tell us your secret. 🙂

1 ReactionBefore you see the rheumatologist pull up the screening test questionnaires for psoriatic arthritis. Skin psoriasis can be as subtle to diagnose as scalp scale and itch.
Are your nails pitted or ridged?
The PEST scale and PASE questions can give you points
for a diagnosis. Tendonitis and family history of autoimmune disease are all important info.
Good luck working for a diagnosis to start treatment.

Mine have all been through Mass General Brigham.
I hate to say it, but the majority of my care there has been substandard. I just had a punch biopsy to check for connective tissue disorders and Behçet's and it went to a depth of 1 mm. 5 to 7 is the standard. The report was, needless to say, "Inconclusive." They also didn't do immunofluorescence staining, which is the best means to check for many connective tissue disorders, like lupus.
At this point, they are either comically awful or they're trying to get me to move on. So I guess I should move on.
Ah ok. I didn't understand what you were asking because every time I've gotten an ANA done it's been via Quest and they've always shown the titer and pattern. That's ridiculous that any lab wouldn't give that info!
