Polycythemia & Bone marrow Disorders screening

‏hello everyone good morning And it’s great to talk to you.At the beginning of 2022, I began to feel terrible pain in my stomach. I went to an internist, and he made me underwent cbc test, which showed that hemoglobin was 20. He ordered me to see a hematologist, who in turn for 2 years made me underwent a lot of exams for the heart, then the kidneys, then the liver, and hepatitis viruses which all came back normal .And the lung,he also asked me to donate blood more than 12 times in 2 years and he gave me a lot of time after every blood donation to follow up the hemoglobin.and he made me underwent these tests:
‏JAK2 V617F & JAK2 Exon 12 which came back Not detected
BCR ABL not detected also
MPL & Calr Mutations Not detected also
‏the problem is that he told me that I must undergo a bone marrow aspiration & biopsy to make sure that there’s no disorder in the bone marrow because there is no secondary cause for high hemoglobin, and that hemoglobin always goes up after a while from the blood donation and the spleen is moderately enlarged, and that I am not Smoker.
I am terrified of undergoing a bone marrow aspiration biopsy,What should I do?

My oncologist said the same. If treatment will be exactly the same either way, no need to endure the bone marrow test and the COST of it.

I had one done at M.D. Anderson in Houston. They put you to sleep. No Pain. THE blood work the day before was worse. I was skipping down the hall when it was over. That was 12 years ago and I am now in Constant pain and they can't find out what is wrong. Right now I take a B12 shot once a week for28 days. I am waiting to die.

Hi @cherren11. I’d like to welcome you to Mayo Connect though I’m certainly sorry to hear you’re not feeling very well, You mentioned having a bone marrow biopsy 12 years ago so you must have had symptoms that led to needing one. It’s good to hear that was a painless event for you but was there anything found on the biopsy? Were you diagnosed with polycythemia vera?
What type of pain are you having now that’s causing so much distress?

I had my bone marrow biopsy&aspiration done last wed,I was a little afraid because it was my first time going into surgery room in hospital so I asked the anesthesiologist to put a lot of spray on my skin before the needle,he told me calm down and he inserted a cannula in my left hand vein and inject 2 anesthesia needs,and when I woke up in the recovery room I felt like I’m traveling through time 😂,no idea what happened or where am I ?,and for 30 mins after I woke up I wasn’t know whether I’m dreaming or real .and after 4 hrs I went to home ,I still feel pain in the area, but it is almost moderate pain not that hard .Yesterday, a pharmacist cleansed the area and put a bandage on it. Results will be available within two weeks.

Hello @mahmoudbakry! You’re making me smile first thing this morning with the story of your biopsy and the experience with the anesthesia! 😅 I know you dreaded having this done and now…pffft, it was a piece of cake!
The discomfort should diminish over the next few days. Now the hard part waiting for the results. Will you let me know what you find out please?

Mahmoud: your experience with the anesthesia made me smile. My 1st 2 BMBx were done with anesthesia because I was afraid of the pain. The next 2 were done without anesthesia because I don't like to ask family members to take off of work to drive me to and from work. Once they get enough local anesthesia to the site, you really don't feel anything. Hope to hear back on your results.

Mahmoud: your experience with the anesthesia made me smile. My 1st 2 BMBx were done with anesthesia because I was afraid of the pain. The next 2 were done without anesthesia because I don't like to ask family members to take off of work to drive me to and from work. Once they get enough local anesthesia to the site, you really don't feel anything. Hope to hear back on your results.

Oh why not ? The family should be with you in this ,because I think local anaesthesia is little bit painful
