Anyone with CKD & type 2 diabetes taking Farxiga (dapagliflozin)?

My endocrinologist suggested I take Farxiga to help with Type 2 diabetic issues and protect Stage 3B from becoming more serious. She is a great doc and follows the science of this drug. Several months after taking the drug, my outside of Mayo nephrologist back then questioned the validity of the drug. So I went to WebMD online, which is not a truly scientific source for information, say that Stage 3B should not be taking the drug, so I stopped last November without the good wishes of my endocrinologist. Fast forward to past February, I see a New Mayo nephrologist who strongly concurs with the usage of Farxiga with my endocrinologist. Both explain that it is the science that encourages them with CKD. When I stopped taking Farxiga, my GFR was 45 and 44 respectively. I restarted Farxiga around March 1st, and my latest GFR was 37. Granted I have had fluctuations in the past, but unsure of the future GFR direction. I will be tested again next week so I will have a better idea. In the meantime, are there any of you who have taken Farxiga and can share your results as it relates to CKD? I know some have talked a little as it affects heart problems or side effects, but I saw very little on this forum with CKD.

I am a female in my 60's with a 30 year history of type 2 diabetes and CKD. Nine months ago my A1C was 11 and GFR was 32. Started on 10mg of Farxiga and AlC is now 6.2 and GFR 50! Have had no side effects at all with Farxiga. Have been unable to tolerate most other diabetes meds due to severe side effects. (primarily nausea and emesis).

My Nephrologist started me on Faxiga on 9/11 had lab drawn on 9/26 and GFR was down 5 points. She said this was normal as it sometimes drops when drug is first started. I have an appointment November 12 lab will be drawn before the appointment if my GFR rate is dropping more I will stop taking it.

Nephrologist started me on Farxiga been taking it for 3 wks GFR dropped 5 points now wants to start me on Kerendia along with the Farxiga I will be taking both. Is it safe to take both drugs together has anyone taken both drugs together. Should one of the drugs be discontinued first.

Farxiga has been highly recommended by my nephrologist and endocrinologist for me. I’m type 2 diabetic and CkD 4a. I have been on Farxiga for about one year, and I have not had any problems with the drug.

@westutx My story with Farxiga was totally different. There can be serious side effects for Farxiga for some, but both my endocrinologist and nephrologists suggested for many that this drug was useful. First, there are 2 doses: the diabetic dose of 5mg and the kidney dose of 10mg. I took the diabetic dose and two events happened for me. There was constant urination throughout the night and my eGFR for both Cystatin C and Creatinine went down quickly. Yes, this can cause dehydration. I used Farxiga on two different occasions over 2 years with the same result. I then created a lab result spreadsheet for my Mayo doc, and he was surprised but accepted it. Later on, I went to another nephrologist and he would not accept any sales people coming to see him about Farxiga. He told me to stop taking the diabetic dose. My endocrinologist was furious as she prescribed it. The bottom line is that it works well for many, but the numerous side effects and kidney loss for some are quite apparent.

I have been taking Farxiga for proteinuria but it has decreased my GFR from 80 to 70. In fact, I have a feeling that I may live longer with CKD without Farxiga. I am thinking about taking herbal medicine and stop Farxiga.

@zukertort70 Welcome to Mayo Clinic Connect. Have you discussed this with your endocrinologist, yet? Is the proteinuria a result of diabetes? If it is not, you may very well want to find the root cause of that before moving forward.

Is this drop in eGFR a one time thing, or a trend? That is how we look at changes in our kidney function. Dehydration can be a big cause of changes, as can time of day the labwork is pulled, different machines processing samples, exercise just before labs, etc.

It is a trend thing. Three years ago my GFR was 85 but now it has reached 75. Basically, gradually but steadily my creatinin level is rising, for example nine months ago it was 1.10 but now it is 1.21. I suffer from IGA nephropathy according to biopsy with my current protein 24 hour urine at 1500 mgdl. I was prescribed prednisone for six months which brought down my proteinuria to 800 mgdl but raised other minerals in my blood. I have been on ramipril as well for six months but my protein 24 hour still vacilates between 1200-1500 mgdl and now my nephrologist has put me on Farxiga for three weeks and I am non-diabetic. However, I feel all these medications are doing more harm to my kidneys than good and I am considering giving them up completely and resort to herbs and fish oil instead.
