I have never used a vest. But I have heard that being able to adjust the amount of vibration is very important. I am far to thin to use a vest and find the Aerobika works well for me. I wonder if a vest is good for lower airway congestion while the Aerobika works well if you have upper airway congestion?

I had the same question ..... what targets the lower airways and what targets the upper. The rep at ElectroMed, the maker of my SmartVest, said the vest targets all airways, lower as well as upper.
I use nebulized saline along with the Aerobika when I use the vest. Even though I have nebulized Levalbuterol, I tend to use Levalbuterol inhaler before vest therapy as that lessens airway clearance time.
