Chronic Post Nasal Drip

Hello; I am a 46 yo male. I have had Eustachian tube since age two. 40+ typanostomy placements bilaterally. 4 cholesteatomas remove with tympanoplasty, twice each side. One year ago I started experiencing post nasal drip. It led to bronchitis and then pneumonia. My ENT sent me to their allergist, I was mildly allergic to grass. Started two nasal sprays, a steroid and an antihistamine. There was no improvement. ENT did a CT of my sinuses found blockage and performed a septoplasty, and opened my turbinites. Surgery did wonders for my breathing; however the post nasal drip is still happening. I am constantly sniffing stuff at the back of my throat/nose into my throat. It is incredibly thick (consistency of rubber cement) which then gets into my upper airway causing me to constantly clear my throat. The only relief I get is if I lay my head over the edge of the bed, fill my nose with saline for 5 minutes then blow my nose and repeat. This alleviates the symptoms for 10 or 15 minutes. The water that is discharged from my nose is clear, however if you reach your hand in the bowl you can grab a handful of clear mucus. It bad enough that if I am not constantly nibbling on something I get sick to my stomach. It annoys everyone around me as expected. It also gets better at night when I am laying down. I use a CPAP and I don’t wake up feeling like I have PND; however within 5 minutes of standing up It starts up. I have a humidifier in my room as that was a suggestion. Has anyone else experienced this?

i have PND. but mine is caused by Chronic Sinusitis. i also had polyps. which my ENT removed twice each nostril. now i am on Dupixent twice a month. been on it for over a year now. works great for keeping the polyps cleared up. but i still have the C. Sinusitis.

If you had chronic rhino sinusitis , why would your ENT only perform septoplasty/turbinate reduction?

That would not address your sinuses.

Usually someone with “CRS” is treated with oral antibiotic/oral steroid taper. 21 day antibiotics w/ a steroid taper.

Was this never offered to you pre surgery?

The lack of healthy mucociliary clearance from your sinuses, is causing the inflammed tissue/thickened secretions to drain down your throat. Typically these secretions will also run past the ET opening and cause inflammation as well.

Thank you for your comment; that particular treatment wasn’t offered; but it may be because I had just finished up two back to back antibiotic and steroid combos for pneumonia; Antibiotic treatment would have equalled around 21 days, the steroid was likely 10 days if I remember correctly.

interestingly no. i have always thought & felt i had allergies. but after testing and being negative. but there are lots of times i still feel that i have allergies. such as in the summer with fresh cut grass🤷‍♂️. but i was put on Dupixent because of the Chronic Sinusitis and the polyps that tend to go together.

Antibiotic treatments are between 15- 21 days. depending on how severe it is. the steroid, usually Prednisone goes 10 days or more. also, depending on how severe the situation.

Hi. I had same stuff. Chronic cough. Caused problems with breathing. Vocal cord dysfunction. Tried it all. Eosinaphilic esophagitis. Lots of problems. I have been on the Fasenra injection pen for 2 years. All those problems were solved by the Fasenra. Pulmonologist and ENT were surprised. All gone and much better. My experience is similar to yours and I've been on 300mg Dupixent injections for about 1.5 years.
I was diagnosed at Mayo clinic Rochester Mn. in 2014 after 7 years of steroid injections and two sinus surgeries. Diagnosis: Aspirin exacerbated respiratory disease. (AERD) Also known as Samter's triad: Asthma, Nasal Polyps, Sinusitis.
All allergy tests were negative at two different Allergist's.
However; a integrative health specialist found I had food intolerances on top of the AERD. So I can not have Gluten, Soy, Corn, fermented foods or alcoholic beverages. No beer unless certified "Gluten free". Ingesting these foods causes: asthma typical reaction, runny nose and or post nasal drip, especially at night if I have accidently been contaminated at a restaurant.
Dupixent has been wonderful, however, about 6 months ago, some of the symptoms are returning. Voice impairment, post nasal, caugh. I do use Budesonide, sea salt sinus rinses that help with post nasal. (Squeeze bottle works best).
You might request an Aspirin challenge for AERD. Also, see a doctor that tests for food intolerances, which is different than an allergy.

curious about the voice impairment. what happens?

Voice becomes whiny according to my wife. Very difficult to sing and hold a note. Rather annoying. I think it's inflammation of vocal nerve.
