What are treatment strategies for reoccurrence of prostate cancer

Posted by mike72 @mike72, Jun 24, 2023

I am a 73 year old diagnosed 2 years ago …Gleason ( 3+4) pelvic mri indicated indeterminate EPE…not in lymph nodes no metastasis…treatment plan was RT,Lupron and Casadex….all is good so far no detectable PSA.
Looking ahead if biochemical reoccurrence happens …what are my treatment plan choices going forward?( science is changing rapidly and want to listen from others)

My UCLA doctors advised me that when my cancer returns they will probably put me back on lupron and Erleada. I am Gleason 9 CRPC had 13 months of lupron and Erleada with RP in the 6 th month. Been off of meds for 6 months and now my psa is .01 and testosterone is 96. The surgery and meds were not bad but not great but now feeling great. Hope to continue for many years, but who knows? Good luck!

I'm on eligard & zytiga and when they lose efficacy, next up will be a parp inhibitor cocktail, when that loses efficacy i will go to a platinum based chemotherapy like docetaxel and when that loses the efficacy i will lean into theranostic nuclear medicine like pluvicto. I'm 52.

2010 proton radiation therapy at Loma Linda..2.5 months of morning radiation (PSA 6.4-Gleason 3 + 3). Five years, no PSA readings.

Since 2015, PSA has increased slightly each year. Currently as October 2022, PSA at 4.29. Gradual increase in PSA. Readings indicate possible recurrence or just increase normally. Very hard to determine at this juncture.

Excellent health aside from the PSA "bump" up!

I have TriCare for life, so I can go anywhere, but currently use the VA system in Florida.

Anyone with a similar situation is urged to comment.

After 40 rads in 2018 and the rad L2 in 2020 back on Zolodex Then in 2023 Jan on Xtandi and now added Xgeva Goolge them 78 PSA back to 1.7 from 9.8 in Jan

Get on Zolodex early and stay on it is my advice
