Severe shoulder pain

My almost 90 year old husband cannot have surgery - has severe shoulder pain. He exercises with small weights, takes Tylenol - increasingly to no avail. Any advice?

Has he gotten trigger point injections from an orthopedic surgeon they might help but won’t take away all pain. It’s very hard has he been evaluated by a doctor of physical therapy

Yes / has had some pt and does exercise.
No injections
