Looks like a TENS unit. These types of things may help minor back pain, but not likely to help with severe,chronic pain. I would not recommend putting your hard earned cash out for this thing, unless it is cheap enough to try and then say, "Oh, well. Didn't work, but at least I only spent*** on it". Maybe they offer a guarantee of some sort?
I decided to check this SAM 2.0 out. It seems to be more legitimate than just a TENS unit. Uses ultrasound. Here's a link to an article about it receiving FDA approval:

Thank you. I used it last night and it did seem to make one area feel better. It has two applicators though. The area where I put the second one actually feels worse. But I did sleep directly on that hip part of the night which may be the problem. So the jury is still out. Another negative is that if your pain is from your back and you have no one to help, it is rather messy and hard to apply. And the picture in your article does not show all the wires that must be attached. But as for self application, practice makes perfect:)
