Results of DEXA after 1 year Evenity

I finished Evenity 12/25 and did a DEXA today. I have not seen the provider.
Results: 1/7/26 Lumbar Spine T-score -2.9 . Prior 11/21/23: -3.8.

1/7/26. Neck Left Femur T-Score -3.5. Prior 11/21/23 -3.8

There was for some reason no report of the right femur. The prior 11/21/23 right femur T-score was -3.5.

Trabecular bone score was not reported 11/21/23 but was reported 1/7/26 and was 1.154 which means degraded microarchitecture. This seems to be a newer element of reporting as I have not seen this on my prior tests.

The density of the left femur increased by 9.1% which is a statistically significant change. (I don't know where the radiologist got this from; it seems incorrect to me. I don't see a 9 % increase in the left femur-it went from -3.8 to -3.5 which is not 9%. I am going to email my provider about it)

So some good results for lumbar spine but not much change on left femur and trabecular shows the worse reading (degraded architecture).
The relative risk fracture is 7 times that of a young healthy adult (compared to 14 times for 11/23 report.

I feel the results are pretty good, not as good as I had hoped but I will discuss with my provider in a few weeks. I did not have any issues at all with Evenity (no side effects) and am overall glad I started treatment. I am not sure what is next: Reclast has been mentioned but I will find out. I plan to continue treatment. I hope to be able to repeat Evenity at some point also.

@normahorn Thank you for your reply. The density value (g/cm2) from 11/23 on the total left femur was 0.526 grams per square inch and the density value (g/cm2) from 1/26 on the neck of the left fetus was 0.551 grams per square cm. I'm still not seeing a 9.1% change here. Can you explain?

Some radiologist needs to do some serious explaining. I calculate an almost 5% increase.

@normahorn Thank you for your reply. I wonder too if the radiologist made an error. I will discuss with the provider and update the post with what I find.

Those are very nice gains, especially at the lumbar spine. The femoral neck improvement is more modest (expected) and moving in the right direction. If both scans were done on the same machine, you might want to ask your doctor whether a TBS reading can be obtained retrospectively from your 2023 scan.

Do you also have total hip results?

If i were in your shoes, I'd be sure to let the doctor know you're interested in repeating evenity down the line. Depending on time horizon, antiresorptive options other than reclast might be worth exploring. And if reclast is the plan, it's timing and dosing are worth discussing.

Keep us posted on what you and your doctor decide.

Thank you for your reply. The hip numbers were not on this report. This Dexa was at the same facility so I will ask the provider about possibly getting TBS reports from 2023.
I appreciate your comments and will post after seeing the provider in a few weeks.

I also have not had any major side effects from Evenity and I would definitely do again. I have 3 more injections until finished. My doctor seems to prefer Prolia before Reclast but I have an appointment end of month to discuss.

Thank you for your comments. I think I prefer not to do Prolix for reason mentioned on these posts. I have an appt. in 2 weeks to discuss this with my provider and will update my post.

@deborahla - Was there anything else you did intentionally to address your OP during this same time period? Resistance exercises? It's so tricky to compare outcomes when many of us are modifying exercises, supplements, hormones, diet, etc in addition to the drug. My understanding of Evenity is that it performs better in the spine that in the hips. Congratulations on your progress!

I have always exercised regularly (walking, biking, pilates) and taken Oscal. Other than that I did not do anything else different. I have a family history (grandmother, mother, sister). I am 72 years old.
