PSA rising every month and is now 5.4 - Stage 4 Metasasized
I've been on Apalutamide and get Eligard shots every 4 months for the past year. My PSA was going down after 4 months, but is now going up every month. I thought the hormone therapy was supposed to work for a few years...It only seemed to help me for 6 months.
Now what? Anyone else have this happen? Is Chemo the next step?

It sometimes happens that a particular medication such as apalutamide does not work for a given patient or works for only a short time. In that event, the patient can be switched to a different similar drug such as Abiraterone/Zytiga or Xtandi to see if it is more effective in that patient. Ask your urologist or medical oncologist what he or she recommends. Good luck.
I'm sorry to hear your news. You must feel discouraged right now.
Chemo is one possibility, but there are many others these days, including brachytherapy (radiation in liquid ingestible form that can target widely-scattered cancer cells).
I hope you'll feel better after a long talk with your care team — you've barely scratched the surface of treatment options so far.

Is chemo next C only in spinal bones hence Xofigio

Every month I get the PSA test and then I'm overwhelmed with worry about what is coming next...
Hi, I have been given Abiraterone & Triptorelin for only 8 months & PSA went down to only 1.5 & steadily risen to 6.5. Pet scan this week so we shall see how my MCSPC is going as not sure at what point I become CR? I am expecting Chemo or targeted therapy to be added to my regime but guess we are not where we want to be but new treatments always give hope. Keep the faith
I am glad you have an appointment with your oncologist so you can discuss your concerns and talk about what comes next. As northoftheborder said above, there are many, many other treatment options even if the class of medications including apalutamide isn't effective. As he said, you have barely scratched the surface of the treatment options available to you. Hang in there and please keep us posted.
3.7 Years in. My PSA is rising. Oncologist is on it. Appointments have been made and treatment plan changes are in the works. Feeling good about that. I've been at this long enough to understand the value of tracking our PSA labs and making treatment decisions based on them. Thank you. Our stories help each of us. We are all learning how to deal with Prostate Cancer. Revealing our stories contributes amazingly to our healing. My cancer can't be cured, it can only be treated. I get it. But, healing comes in many ways. We can heal ourselves meaningful ways on this journey. .
Im not on ADT , however some of our friends are. From time to time I hear one ADT drug does not work for various reasons. We have so many different types of drugs these days. You will have to switch to another one . Let us know what your Dr puts you on. Many could learn from this , as well. James on Vancouver Island .
