Post surgery erectile recovery.

My surgery was 3 years ago and was nerve sparing and apparently successful given my continued insignificant PSA tests since. My recovery period was quite long and uncomfortable but ultimately physically and emotionally very successful, with regular reviews with my surgeon and a supervised pelvic floor exercise regime that has enabled a continued 90% avoidance of urinary leaks.

My only incontinence risk event are when I’m sexually aroused and heavy cough or flatulence without first consciously clamping my pelvic floor muscles.

Ok ..erectile loss was pretty much total for 6-9 months and has never really returned in terms of being naturally erect enough for penetration. My doctor recommended the use of a penis pump and prescribed 5 mg of cialis daily for about 2 months, then changed to 20 mg when ever I desired an erection. Later I was switched to Viagra 50 or later 100mg for use when I described an erection.

The 20 mg cialis gave limited but barely useable results. The 100mg viagra allowed a greater result but not lasting and gave me significant blocked nose and flushed face effects.

Finally I was prescribed Quadmix injections on an as needed basis (never more than every 2 days) which remains fully successful @ 40 units. My sex life is restored.

A recent YouTube specialists video alerted me that I had never been properly guided in the use of the penis pump as an ongoing therapy rather than a precursor to producing an erection for sex. Similarly the medications were always presented as a precursor to sex rather than a therapy.

Question .. would it be beneficial even at this stage to perform regular penis pump therapy as an aid to erectile health? Should I retry the cialis 20mg with the pump therapy?

I use the pump prior to injection so I have a larger area to inject in the right spot , happy for you the injections work , I’ve been using various mixtures of Trimix and Bimix with poor results if my erection is barely big enough for sex I have no feeling which the surgeon did not have a good explanation for. He said nothing he did during surgery would cause that . My age at 79 may be a factor but I was totally active a year ago before surgery having sex 2-3 times a week. I hate to give up but I’m more depressed when I try without results

...on a video from the PCRI.org Mark Scholz MD, a 20+ year medical oncologist was incensed at surgeons that did not recommend early intervention post surgery. After several weeks of inactivity most men should start injecting the penis to maintain muscle function. He mentioned even with successful nerve sparing the nervous system may also have been shocked. The muscles must be supported until that neurological effect resolves. Waiting till failure is obvious is not smart. Pills and pumps may play their part further down the path to restoring function

I'm not a medical professional, but rather just another RP patient layman so take what I say with a grain of salt. I was 70 years old 15 months ago when I had RP. I'd suggest trying to find a good male sexual health doctor or at least discuss this with whoever you are seeing. I traveled out of town for my surgery but I continue to see my local urologist for his sexual health advice. He has me on a daily 5 mg tadalafil (for the nocturnal erections while I sleep) and he also has me use 100 mg sildenafil (an hour before on an empty stomach) when I want to have sex. He initially said to use 5 mg tadalafil every day and increase it to 20 mg of tadalafil on the days I want to have sex. I tried that, but ended up (as advised by my doctor) using 5 mg tadalafil every day and supplement it with 100 mg sildenafil on the days I want to have sex as that worked better for me. Recovery was very slow but I'm now (15 months after surgery) fully back to pre-surgery sexual performance. But I expect to be on the pills for the rest of my life as I don't do as well without them and I don't have any bad side effects from them. My doctor didn't mention it, but I also took a good multi-vitamin supplement and a magnesium supplement (I get at Costco) since I don't eat good enough to get my RDA from the foods I eat. I don't know if that helped or not, but it seemed like a good idea to make sure my nerves were getting all the basic nutrients they need. Bottom line: I'd keep trying therapy until a good sexual health doctor told me it wasn't useful to keep trying. From what you said I'd probably try the daily 5 mg tadalafil and increase to 20 mg tadalafil on days I was going to try to have sex. I'd probably give the vacuum pump another try as well, but I'd really suggest discussing all this with a good male sexual health doctor. Just my 2 cents as a layman (ie. not a medical professional). Also, I found expired sildenafil to not be as potent as fresh stuff, so I'd suggest getting new meds if you happen to have some old meds still around from before. You can get both tadalafil and sildenafil pretty cheap these days using a Goodrx coupon or at Costco with member discount. Best wishes.

@retiredguy has hit all the high points. Daily Cialis is recommended to keep the blood flowing properly in your penis. Using the pump will not get you an erection that’s good enough to have sex, but it keeps the tissues in the penis active so that it will work correctly When You use Viagra. Get the generic Viagra, 20mg sildenafil. and you can pick how much “Viagra” you actually need?.

It’s good to hear you found an Injection that works for you. They make bimix and Trimix Injections when the single drug doesn’t work sufficiently. Just something to keep in mind in the future, if you have issues.

I take the daily cialis and days I don’t inject take 100 mg of Viagra no luck

I am almost 6 yrs since RP. My wife was not well this whole time. My choice was to do nothing. No stimulation. I would occasionally get a nighttime arousal but pretty weak. I went to my urologist at Mayo Phoenix in the last few months. He indicated it is a use it or lose it and implant surgery might be our only option. But we should try all the steps first. Daily cialis has given me some stimulation but not a useable erection. The 20mg before sex is not working either. Neither has the 100mg viagra. I went for the medium strength trimix injections. 5 units was starting dose. I have used three times and 5 units causes a full erection for about 2.5hrs. Injections are not ideal but the fact that I can get a useable erection has been more than I expected. my orgasms have been a little different.

I wish I could help with this. I am only 5-months post-op. I have managed two orgasms with stimulation, but had no erection. All stimulation was focused on the nerve center on the underside of the penis. And...I ejaculated URINE. "OMG"...ultra embarrassing. That is the only reason I am commenting here: If you plan and hope for a nice sexual experience, I HIGHLY recommend that you urinate immediately before you begin your activity, in order to avoid the embarrassment of ejaculating urine. This might include the need to wear a condom, but condoms make stimulation nearly impossible...it is like adding many layers of skin to your penis which is already difficult to get fully erect. BTW...or me, my 5mg Cialis is a joke...it does absolutely nothing. Perhaps I should ask the doctor to increase it to a safe concentration. Good luck...I have pretty much resigned myself to not ever having vaginal-penetrating sex again. That is hard to bear. And BTW again...has anyone who is single/unmarried out there, had their physician write an order for sex therapy...sex therapy with a real surrogate partner? Did it work for you?
