Penile Duplex Ultrasound

Posted by ohioman @ohioman, Dec 17, 2025

Hi all, Scheduled to have a penile duplex ultrasound tomorrow morning. Anyone have that done - and if so, any questions I should be asking the doctor or other guidance?

You will get an injection into the side of your penis almost painless such a small needle. They will be hoping to induce an erection to see if the blood is able to come into the penis, and the percentage that remains trapped to maintain an erection. In my case leakage was so bad never got nothing not even a chubby. Ended up with penile implant in the end best choice i ever made. Make sure to have the dr explain clearly the results and options.

@arctic22
Thanks for the insights. Much appreciated.

Did the test. Turns out flow in (Peak Systolic Velocity (PSV)) - is running less than half of what is the average healthy make inflow should be - one side at 14 cm/sec and the other side at 11 cm/sec. Suggests arteriosclerosis- humbling and eye opening. End Systolic Velocity good - about 1 cm/sec.

Bottom line - insufficient flow coming in to create solid erections, though blood still be trapped ok to maintain what’s there.

We’re going to experiment with 10mg of Tadalafil daily for 3 months to see if that makes a difference.

Shots still an option, as the shot did give results that were better than my organic baseline.

I tried the shots got no results. If and when you decide to go to a implant look at a site called franktalk ..become a member it's free anything and everything you ever wanted to know about penile implants very helpful.

I had this test done well over ten years ago, and they found poor arterial flow, as opposed to venous flow. One thing that was funny at the time was after the tech injected me, he handed me a men's magazine to help achieve an erection. I basically told him I was waaaaayyy past looking at men's magazines but he was just doing his job. The results led to my use of Trimix which has always done the job of achieving a very good erection. Unfortunately, I had a radical prostatectomy three years ago which was supposed to spare the nerves that produce erections, but that aspect failed. Now, Trimix still leads to an erection, but without those nerves adding some arousal blood flow, it just isn't the same. Climaxes are very, very challenging.

@tomf Your experience brought a smile. AND kind of made me wonder if I should have stayed quiet and kept my eyes closed and perhaps just more relaxed during the process. I felt like Curious George goes ot the urologist and wanted to watch the ultra sound screen as he was taking his measurements, etc. Mental distraction from what is just a usually happy feeling with an erection that may have worked against the biomechanics of incoming arterial flow. (Though perhaps the chemical is such a strong override that what I'm thinking about has no impact?)

I'm heartened by your earlier experience with Trimix, though sorry to hear about current struggles.

I may just go for the shots when I next meet urlogist (actually his NP) in March. Part of me feels skiddish ... and another part says ... go for what you can while you have what you have.

(I've got PC, though 3+3 Gleason so pursuing active surveillance. BPH still an issue for me, after PAE this past July)

I recommend you take the shot of Trimix sooner rather than later. It is usually very successful. I just had to find the right dosage to achieve the desired results. PC has nothing to do with it. 3+3 is easily controlled with radiation and only mild side effects. Good luck.

Thanks for the input!

2 more questions:

1. Since the results from my ultrasound would suggest arteriosclerosis -- have others had this test, THEN found there are more things they attended to for overall cardiovascular health?
2. Any have experience with using 10 mg/daily of Tadafil? Suggestions for getting the most out of it? (Time of day to take, etc.)
