Overwhelming lumbar spine pain; most of the time I can't sit down.

In 2022, I fell hard onto a concrete floor. I slipped, went airborn, and slammed down onto a concrete floor; my lumbar spine was the point of initial contact, then thoracic spine hit, and then cervical.

MRI of pelvis and sacrum (June 2022) revealed bulging disks. No broken bones. No contrast was used. Would using contrast show/highlight anything that a standard MRI might miss? I currently use lidocaine patches daily for a little relief and Tizanidine, 4mg 3x/day for muscle spasms throughout my back.

What type of physician (what specialty) do I see that could possible help me? I have a neurologist who reviewed all of my films, reports etc. and she saw nothing in my films that would explain my pain.

Does anyone have any insight into what I am describing?

Cause (fall) and effect seems clear. I would try another doc perhaps an orthopedic to take a fresh look at MRI. Your note suggests you are still functional. What is your pain level? If your pain level in on the low side I would not be too aggressive. Maybe you just need more time to heal.

Thank you for your reply. Pain levels are high. I begin my day with no lower than a 7/10. By lunch, I am 10+/10 each and every day. I can't lay on my spine. I have to sleep on my side. If I roll onto my back, I wake up immediately from the pain. I can't sit, at all, without pain. Getting into and out of an automobile is overwhelmingly painful. My ambulation is affected. I am at a loss. I have a PT program for home that I do..... I feel I am progressively getting worse.

Sounds very painful. You may want to try an orthopedic doc and sit with him/her to review the MRI...It gives pretty clear pictures. Have you reviewed radiologist report?

I found help with an orthopedist who does not do surgery. If your imaging was too soon after the fall, you might want to redo (x-ray would probably suffice) to make sure no fractures. They show up late in imaging. I went through 6 months of pain with MD's not listening and a specialist redid x-rays sure enough, fractures.

I have a PT who doesn't do exercises with me, only massage and ultrasound, and she is gifted. I am lucky because after maybe 6 months of pain like yours, I have been able to walk and walking for some reason helps. I also stay in bed a lot, especially if I make a mistake, because my back is better sitting in bed with lots of pillows behind.

I cannot sit against anything. Which makes me think you might have fractures (I have 7).

Hairline fractures can be missed even with a contrast MRI. The primary symptom of hairline fracture is pain. If contrast doesn't reveal any source, insist on a CT. The inability to sit leads me to the coccyx. While you may have impacted the sacrum first, more likely the tailbone. Sometimes this is an easy fix.

Maybe you could try a CBD ointment/salve in the area. It worked for me, but some people get no relief with it. You need a high concentration, not anything under 1000 mg. It's expensive, and only lasts about an hour or so.
Much better than the lidocaine patches.

Did you have an MRI on thoracic? The spine is tricky and there can be referred pain. Just wondering if it can be from further up

So sorry to hear of your terrible pain. There is no doubt something more going on. I too have had back issues most of my life and have had 3 fusion surgeries, which I would not recommend until you have exhausted all possible alternative options. I agree with others that an Orthopedic doctor would be good or a Pain, Medicine & Rehab doctor as they can do amazing things with various injections, etc. I would also suggest becoming well educated on understanding your results from the MRI, X-rays and any other results. To assist you I would highly recommend reading the book, "Back & Neck Health" by my Mayo Clinic (Rochester) Neurosurgeon, Dr. Mohamad Bydon. This easy to understand book may help you in how to positively move forward with a treatment plan.
