Osteopsorosis and Medication

Posted by codythedog @codythedog, Apr 16 10:56am

Hi,
I have been working very hard the last four years with strength classes, bootcamps, etc. and calcium supplements and just got another decline in my Bone Density Test. I feel frustrated but want to continue an active lifestyle without fractures. My medical history does consist of dental issues and I am very concerned about going on the drugs.
I would love to hear about anyone having these comorbidities and if they found a good combination.

Codythedog, even without seeing your numbers, I'd suggest Tymlos or Forteo. Neither of these medications complicates dental issues. They do build the most resilient bone.
There are few medications for osteoporosis. It seems like there are many because the bisphosphonates have many generic names. You might avoid them with dental issues. One of their side effects in some is osteonecrosis. This is usually after many years of use.
There is Prolia the one that provides a trap for people with dental issues. It is the worst in terms of this side effect although many do not suffer with it in that way.
Prolia and the bisphosphonates are considered anti resorptives their main mechanism of action is to prevent the normal process of bone renewal.
Evenity is a medication sort of in the middle. It does list dental issues as a seen side effect. It is primarily antiresorptive and is called such. It adds bone but it isn't remodeled bone.
Best of all are the wonder drugs the expensive anabolic osteoporosis medications. Tymlos and Forteo. No osteonecrosis in their mechanism. They are used to repair the damage to the teeth that sometimes happens with the antiresorptives. They build the best, most resilient, adaptive bone that you need if you plan to remain active.
You can see this is a completely unbiased narrative from someone who doesn't have dental issues. Pay close attention to advice from others with more experience that will follow.

Thank you. I am trying to educate myself as much as possible before seeing a specialist later in the year.

I am new to this site and only give you advice I wish I had received even a few years ago while I was active, no issues, walking on treadmill, anything I wanted to do. I knew I had osteoporosis but could not tolerate the bisphosphonates. I was never referred to an endocrinologist or any other specialist in osteoporosis which to me is a terrible downfall in the medical field of patient treatment by a family doctor.
So, without knowledge, I just did as you are doing, exercise, weight bearing, etc and last December I had two vertabre fractures and while in therapy, developed two more. I have now had Kyphoplasty on the last two and all are healed, but now I have constant pain from all that has happened to me that is going to take time to heal. So I am telling you this, that you already see what you are doing is not helping so find you an endocrinologist who will have your blood work done and then will sit you down and walk you through all the options.
My Dr and I chose Tymlos for it works and she says she likes it better than others. Plus, like you, I have dental matters I need to have done in the future and Tymlos does not interfere with that.
Ny score was only -2.9 but that was bad enough to have fractures. I did not fall, I just had been busy bending over a lot and wasn't lifting anything heavy. I just crossed my leg to put on my shoe after being tired and snap.
Trust me, you will be so glad to nip this in the bud before you are where I ended up, thinking I could take care of it myself. Was I ever wrong. Good luck.

Hi,
I have seen a Rheumatologist in the past and moved away from that area. I do have an appointment with an Endocrinologist later in the year. I really want a drug that does not deteriorate the jawbone. Thank you for your information. I am so sorry that you are dealing with fractures and pain.

thank you for all this information as i need to find a safe alternative as I have to have dental work done and my doctor wants me to try Prolia. I just do not trust this one drug. I have read a lot of different things on this researching different drugs.
