Orgovyx & Gynecomastia
Hey, I'm 56, in really good health and will start a 6-month treatment of ADT with Orgovyx. Brachytherapy and Targeted radiation is scheduled for mid October going into late November. I elected to treat my prostate Cancer this route instead of with surgery simply because I was going to need to have radiation post-surgery anyway. I also had nerve involvement with a 4+3 Gleason score - nerve sparing surgery was highly unlikely.
Question, I'm striving to stay very fit as I'm going through this, but am concerned about the reported fatigue, cardio impacts and even Gynecomastia? Has anyone completed 6 month regimen of Orgovyx and come out on the other side with good results? IS Gynecomastia really a hige concern? I go to the gym 5 days a week and walk 3 miles a day or swim a minimum of 5 days a week too. Any advice?

I'm 4-1/2 months in Orgovyx and aside from loss of libido, I have almost none of the other side effects. I'm unable to run or walk briskly as I used to do with my grandkids, but not sure if due to Orgovyx of my age 69. An hour of slow walk, I can manage.
I may be an outlier, but I lost weight while on Orgovyx (six months). I did workout like crazy. My side effects were chills, loss of appetite, memory fog, loss of body hair, anorgasmia (no ED though), insomnia and depression. There were days I didn't feel like doing my usual workout but I did it anyway and had little trouble getting through it. I wasn't able to put on any muscle while on Orgovyx but didn't lose that much.
The worst part of Orgovyx for me has been the recovery. I was expecting a quick recovery but it's now a year later and my testosterone is still not even half of what it was before and is well below normal.
I did 6 months of Orgovyx with no major problems. I watched calories, essentially quit drinking, hit the gym 5 days a week and walked 18 holes twice a week. I did not experience any mood swings or brain fog. Physical endurance did not suffer. I did have total loss of libido. Also shaved a whole lot less. I did have some hot flashes but they were no big deal. Oh.....golf game still sucked......
I was 68 at diagnosis, low volume 4+3, Decipher .81, PSA 7.2, PSMA PET was clear. Barrigel spacer placed SBRT (36.25 Gy with focal boost to 40 Gy at primary lesion) finished Dec 2023, Orgovyx finished June 2024.
Today I am hard pressed to know that any treatment was done, and latest PSA is 0.47.
Compared to others with cancer this has been and easy ride.

Almost 4 years on ADT (Firmagon, then Orgovyx) and Erleada. Yes, gynocomastia happens. Wear loose t-shirts and/or call them your "pecs". 🙂
In my case, it was helpful having trans friends and acquaintances, because their experiences taught me that having the wrong body doesn't change my gender. I've lost much of my body hair, I have some minor breast development, I shave my face only half as often, and my whole body is more smooth and rounded than I used to be, but none of those prevents me from being male. It's just who I am.
Hey man, thanks for the heads up. Tell me about depression and insomnia? Did it creep up on you? How did you deal? I'm reading that good cardio once or twice a day should take the edge off the mood swings and depression. I lift weight often and will quit drinking once on Orgovyx (its tough on the liver to begin with). I can deal with anything for 180 days - especially to kill this Cancer with a hope and a prayer.
I started ADT last October, one of the potential side effects is Gynecomastia, which I was initially concerned about, my doc tells me it takes about a year on ADT to occur. But I insisted that I did not want this to happen since I am a gym regular and life long weight lifter, he prescribed me 10mg of Tamoxifen to be taken daily. This drug stops increased estrogen production that leads to the “man boobs”. No additional side effects with this drug. Btw, I have continued my standard high intensity weight training while on ADT, and I have not lost any strength after 10 months. You just have to deal with weird side effects and push yourself out of the fatigue every day. Once I get to the gym each day, I feel very normal and never have side effects when lifting.
Thank you - Im supposed to be on ADT for 6 months only, but I will inquire about Tamoxifen - my mother took it years ago for breast cancer actually. Thanks for the gym advice. I will push through the exhaustion - lots of coffee and good workouts.
I’ve been on ADT for nine years. Started off with seven years of Lupron, followed by two years of Orgovyx.
I’ve never had a problem gaining weight.
Never had a problem with gynecomastia, I go to the gym three days a week and about half the men have bigger breasts than I do after all these years, Mine are barely enlarged. It really surprises me how other men look, do they have Prostate cancer???
I go to the gym three days a week, And walk On a track 1 mile in the morning And 1 mile in the afternoon. I walk as fast as I can jogging at times. If I go too fast, I get a lot of skipped heartbeats so I take it a little easy.
