My PSMA PET Report

Hi All, got some not so great (but probably not unexpected ) news.
Scan found a bone lesion and possible a soft tissue area. Starting Firmagon on Wednesday and with this finding maybe additional drug. Radiation starts in late August or early September. Can they zap these areas in the report?

ABDOMEN/PELVIS:

Mild soft tissue fullness and stranding in the left posterior lateral aspect of the lower pelvis subjacent to the pyriformis musculature, image 3/28 with couple of adjacent small nodular foci of increased uptake, max SUV 5.6, new compared to prior study, possibly representing metastatic disease.

Previously seen increased uptake along the left lateral aspect of the prostate gland is no longer visualized.

MUSCULOSKELETAL:

Prominent sclerosis involving the left inferior pubic ramus image 3/315 with increased uptake max SUV 7.7 compatible with metastatic disease.

Tiny focus of minimal increased uptake in the roof of the left acetabulum, anteriorly, image 4/28, max SUV 2.2.

I am so sorry to hear that they found some small additional lesions : (.
I will let experienced people give you opinion and advise and I am sure that they have many good ideas of how to tackle that. Just wanted to say that SUV 2.2 could be some inflammation of a scar tissue if you ever injured that hip before, but regardless, you will have systemic therapy and it will effect all of tiny lesions seen and unseen - so that is good.
How is your post surgery recovery going on ? I hope that you are in no pain by now and that you can embark on a new set of treatments in September with no delay.

Hi Surfer, Thanks! Surgery recovery has gone well. No pain really and only a little incontinence. I have resumed my normal level of activity with no restrictions.
It’s weird, right now looking forward to Firmagon and wherever else they do for me. Helps me feel like I’ll be making progress. I go to disneyworld and Disney cruise the day after my injection so looking forward to that. 🙂

Well, those are WONDERFUL news !!! : ))) Yeayyyyy to full post-op recovery and so FAST ! I am sure that you will finish you "clean-up" protocol with the same success < 3 ! How fun to go to a Disney cruise : ))) !!! I am sure that it will do you as much good as treatments themselves : ))) Our body heals in many ways and with a help of many things - you will beat this thing into submission, I have no doubt 🙂

Sorry to see the new uptakes but glad to see recovery going well. My PSMA PET lit up pretty much from chest to pelvis with a spot on L5 vertebrae. RO told me that the only place he would hit, other than the prostate itself, was the bone tumor if I started getting any pain. He said the meds would have to take care of everything else. Don't know if it's the same in your case but sending good wishes for the best outcome possible anyway.

You were just diagnosed, any idea how your meds are doing? How will you monitor, periodic PET PSMA?

On doublet therapy (Orgovyx and Nubeqa). Blood work 30 days into treatment showed PSA going from 30.11 to 0.55 and testosterone level going from 450 to 10 so I guess they are working. No issues with cardiac enzymes either which, in my case, is a big deal. I’m a heart failure patient as well. I guess so far, so good. Another blood test on 9/1 so we shall see. Thanks for asking. PSMA PET will be either 6 or 9 months after IMRT ends in October. They need to check with the insurance company for allowable timeframes.

Good news Mike! Thats reassuring! Hopefully all that stuff will work in my case. I have heart disease in my family but mine is ok so far. I’m gonna need it 🙂
