Mayo Clinic research survey for people in Florida who have had COVID

Have you had COVID-19 and live in Florida?

Mayo Clinic researchers want to hear from you to understand the challenges people with COVID-19 are facing and how different populations in Florida are affected.

This research project aims to understand the struggles and health disparities experienced by people living in Florida, who have had COVID-19. By identifying and addressing these disparities, the project seeks to contribute to more equitable healthcare practices and policies, ultimately improving the health outcomes and quality of life for people affected by COVID-19.

You can take part in the survey if you:

  • are 18 years of age or older,
  • have had COVID-19, and
  • live in Florida

The survey will take approximately 20 minutes to complete.

The survey is online and your answers remain anonymous. Mayo Clinic is committed to respecting and honoring the confidentiality and privacy of everyone who takes part in this research.
