Masturbation post prostatectomy and radiation. Will it affect PSA?

Posted by knoctor @knoctor, Jun 25 1:05pm

Prostatectomy 12 years ago, and maintained non detectable PSA. recent PSA in 2024 rise rise to .16 and had 37 bouts of salvage radiation. PSA is staying pretty level but rose to ,18. Pet Scans, MRI, negative, no bone metastasis. I have a high testosterone around 500. Under active surveillance now to monitor every 3 months. I know you should avoid sexual activity, bicycling, etc. prior to testing. Would masturbation or intermittent?>? sex between 3 month testing have effects on PSA?>

When I got my first PSA test I asked the doctor the same thing, in case sexual activity could be raising my PSA. He said that it could but, at that time, it wouldn't be enough to go from safe to danger zone. But, now that there's no prostate, I don't see how sexual activity could raise it in any way, any PSA left in your system that is detectable indicates it may be hanging out elsewhere. What's your urologist say?

My PCP and Urologists both asked me if I had any sexual activity before my PSA tests that showed my PSA was rising. They explained that sexual activity can slightly raise PSA levels. But only in rare cases would it be a significant raise. To be safe I wouldn't do anything for about 10 days before your PSA.

I did not have RP only radiation. Two years later my UFHPTI R/O says don't need to do PSA every 3 months but every 6 months. However, my Mayo PCP wants to keep doing it every 3 months. My last test at the 2-year mark was 0.10 Lowest it has ever been.

Those who did have RP can probably answer your questions better than I. However, I am confused. You no longer have a prostate so how does the sex, bike riding, affect you now versus when you had a prostate which those activities could affect prostate?

I have also asked the question and my oncologist says no! It will not raise the psa.

Curious who told you that you should avoid sexual activity, bicycling, etc.? I ride hard 3-4 times a week while on ADT and the doctor also encourages sex and weight lifting. Frankly haven't heard that anywhere.

There are a few sources online that I read after my first high PSA test that specifically mentioned those things. It has been long enough I don't remember where I read that but it was a medical journal somewhere.

I think that it's one of those situation that in some men that some or all of these things may raise PSA - but the level it rises is subject to some debate. My doctor confirmed that all of those things could be true but weren't likely to have any meaningful impact on my PSA.

What I read was all about rising PSA, it had nothing to do with post RARP or ADT or anything whatsoever, I found it when trying to "game" my PSA by hoping beyond hope that the only reason my PSA was elevated was due to these things rather than cancer.

I am confused by the original poster. If I read right he had a RP and has his prostate removed. The things that would irritate the prostate before a PSA test would then not be there as there is no prostate and no rise of PSA from prostate irritation.

I went to Mayo and UFHPTI and both recommended prior to PSA tests to not have vigorous sex, ride a bike, etc. as statistics were inferring could cause prostate irritation and you could get a rise of PSA due to the irritation.

I am confused since I did not have RP what the relationship of these activities have on PSA when you do not have a prostate to become irritated by those activities. I can asked my R/Os and I have a office visit with my PCP to go over my last PSA test. And reference that test I did not ride my bike (I do 20 mile rides) a week prior to my PSA test as was still being guided not to do that by my PCP and R/Os prior to PSA tests. But I have a prostate.

Yeah, it doesn't make sense. Any psa any time after RP I would think would be margin or spread not movement related.

Original poster here. I deduced the very same conclusion, no prostate no effect on PSA, just wanted to verify I was thinking correctly. Thanks for replies

I had a Prostatectomy AND salvage radiation 7 years later. My oncologist told me not to ride a bike a day or two before my PSA test, which doesn’t make sense to me. Any thoughts? He also said , said would not have any effect on the test.
