How to increase my ionised calcium?

Posted by shail @shail, 3 days ago

Hi, my ionised calcium is low 0.98 [1.10 - 1.35] though my total calcium is fine. My nutrition has a good calcium and I take algae calcium [500mg] I am unable to start my teriparatide treatment because of it. any tips please?

Shail, do you have a measurement of vitamin 25(OH)D₃ levels? If those are low, it could be a simple fix--supplement D3. The trouble is that parathyroid disfunction or less commonly pancreatitis can cause low ionized calcium. Is your prescriber following with tests to determine the cause of low ionized calcium.

@gently thanks for responding. u are really knowledgeable!
my calcium is 9.97
my D3 levels are fine at 78 [30 - 100]
my iPTH [intact parathyroid hormone] is also 48.40 [15 -65]
I need to rule out pancreatitis tho all my thyroid, Liver & kidney function tests are fine.
mostly all seems to be good apart from homocysteine being slightly high and ESR 35 [ 0 - 20]

actually doctors just ask for calcium and iPTH - I got ionised calcium tested on my own from tips I received here and on chatgpt. when I informed the doctor that ionised calcium was low he prescribed an extra 500mg calcium citrate. [I will increase my algae calcium]
accd to chatgpt ionised calcium could be low due to shallow breathing, highly alkaline body [ive reduced my fruit intake and need to start pranayam daily] also I was taking 500mg algae calcium so will increase it to 1000mg.

maybe a 24 hr urine test can be helpful if after a month of 1000mg calcium it still remains low.

I wonder why the doctors don't ask for ionised calcium as accd to chatgpt the treatment of teriparatide would be quite ineffective if it's low and perhaps harmful
