How long should pain last after total hip replacement?

Hello. I'm 47 and had a total (r) hip replacement 20 months ago. I keep waiting for the pain and discomfort (especially while active or sleeping on right side) to become more minimal with time. However, this doesn't seem to be the case ...yet.
Has anyone else experienced prolonged pain? Not sure whether to be alarmed or just more patient.

@dana413 Yeah, 20 months does seem like a long time. If it was my hip (I had two THR and three revisions) I would be concerned and seeking answers. At various times over the past 19 years, I have had bursitis, nerve pain, tight ligaments and a few other issues. Each has been resolved with the correct treatment or time.

What have the surgeon and PT had to say about the continuing pain? Has someone physically manipulated your leg to try to locate the exact source of pain? Has any imaging (beyond an x-ray) been done to pinpoint the problem?

Hello I had a torn labrum in my left hip due to a work vehicle accident in 2018, since than I had the labrum repaired than I THR in 2021, I continued to have sharp groin pain when sitting to long than when standing .i had several cortisone injections in that area but they didn’t help much, I had a couple hip reversion doctors tell me I have bursitis from I MRI I had in 2023 they recommended I have I procedure called a iliopsoas released “ the doctor would go back in with a tool to cut and to lengthen the iliopsoas and hopefully by doing so it would help with the pain . I had that done at Mayo December of 2024 so far it seems a little better I have been doing PT since February of 2025 I am getting stronger in the legs and hip but that deep hip pain is just stubborn and does not want to go away so with that said I am having another cortisone injection in the next week or two we will see how that goes but myself and my PT feels this is the best it will get . The surgeon did say it can be a whole year to get the full benefit from the iliopsoas procedure that will be December of this year so we will see what happens . Thank you and good luck

My friend had a labrum tear like you describe about 10 years ago - she always had some level of pain, but now she said it "has faded into the background" as she gets older (past 80) with other issues.

I’m glad her pain faded still 10 years is a long time anyway thank you so much for reaching out to me . I will be in touch with my doctor from Mayo in a couple weeks to discuss further options if any .

I was for the most part pain free after a THR in less than 8 months maybe even sooner than that. I was happy to have been feeling so good and then a dog head butted me twice in my operated thigh. Anyway, 20 months of pain is unacceptable. That's nearly two years. Maybe you should get a second opinion if you haven't done that already.
