Genital sensations that stimulate pain/unwanted feeling of orgasm
I have been experiencing an unwanted feeling of sexual arousal/mild pain in the genital area. It comes and goes but most of the time it happens when I am just relaxing, sitting or lying down. Also, my feet can sometimes feel restless.
I am diabetic for almost two decades. Had spine surgery to fix disc herniation in L4 and L5.
Was wondering if this can be diabetic neuropathy.

I’m sorry you’re experiencing this; I can imagine how distracting and uncomfortable this may be for you. This may or may not be helpful to you, but I had a range of sensations in my vulva following cancer surgery in my pelvic area almost two years ago now. Because my urinary and bowel systems were also impacted, I was referred for pelvic PT. When I happened to mention these sensations that would wake me up from sound sleep or make drifting off to sleep nearly impossible, she assessed for neuropathy. She was quite knowledgeable about nerve pathways throughout the pelvis and how they interact with muscle, bone, tendons, bloodflow, etc. She taught me some simple “neural glide” exercises that, lo and behold, made the problem get better and then go away. It has come back occasionally (usually b/c I’ve gotten out of the habit of using my estrogen cream and the tissues have atrophied), but I can restart the cream and resume the exercises and the situation resolves again. Pelvic PT, if you haven’t explored it, might be worth a look (or a second look). That was a reminder to me, too, that hormone levels interact with all of these systems and tissues as well, so keep that in mind as you sleuth this out. And, just like any profession, there is a range of training, experience, skill and “bedside manner” among pelvic PT’s, so I would encourage you to research your options if this is something that interests you.

Thank you so much for your kind response and all the information you provided. I have been reading up about Pelvic PT but now, will definitely give it a second look. Sounds very optimistic. I mentioned these feelings to my gyno and she prescribed Estradiol cream. I haven't used it but after reading your response, I feel safe to say I will now give it a try. Thank you so much for sharing your experience with me.
You are most welcome, @carleena. Estradiol is the cream I use and only in the first inch of my vagina (as my cancer was estrogen sensitive), so I lean toward coverage around my vulva. (Words I am not used to typing on a regular basis! 🙃) And, since I only use that twice per week due to risk, I use other topical moisturizers on other days…(rotating around with coconut oil, salve that I make and hyaluronic acid/vitamin C serum that I also use on the sensitive skin around my eyes). All of it has helped. I hadn’t thought of the tissue in and around my vulva as needing moisture care before that. I hope to hear that you’ve had some success with finding a helpful practitioner and a regimen that works for you. Best wishes!

Estradiol creme is very very safe and well tolerated, and works very differently than oral estrogen. It's made a world of difference for so many women to repair or prevent what is referred to as genitourinary syndrome of menopause, formerly "vaginal atrophy" Keeping the vulva and surrounding tissues healthy decreases risks of UTIs and difficult complications from that.
