exercising while on ADT

Has anyone felt the benefits of eliminating fatigue while on ADT by regular exercise? I took ADT (camcevi) 2 years ago with radiation. I felt exhausted after 2 hours of manual labor. My PSA is up again and the Dr. is talking about the Embark trial which is ADT WITH ELAZUTANIDE.

I swear by consistent exercise and a healthy diet to combat the fatigue from ADT. I’ve been on and off it for five or six years, and I rarely notice any material fatigue. I try to average 45 minutes to an hour a day, with a combination of weightlifting, cardio, yoga, etc. I am 59 years old.

I found exercise helpful during ADT. The trick for me is to monitor my effort otherwise if I went to hard I would gut myself for a few days and if I didn't go hard enough I would get the benefit. I use a heartrate monitor and figure out the best heartrate to achieve my goal. I've used one for training over the past 30 years as a former bike racer and sports official. I am currently a fitness rider and found the aerobic zone through trial and error during my ADT. A little science goes along way with this.

Yes, with a caveat. On ADT (exacerbated by a spinal injury in my case), there's what I call an "energy cliff". Just enough exercise makes you feel better, but one step too many and you're over the edge of the cliff and exhausted for days. It's just a matter of trial and error. Find your current limit, then push very gently to expand it; driving yourself hard is counterproductive in your new reality.

While the EMBARK Trial did show that there is a real benefit to using Enzalutamide Along with ADT there is a problem with fatigue from both ADT and Enzalutamide. Many people have complained about much more fatigue with it, though it doesn’t affect everybody that way.

A better option would be to get Darolutamide, Which works almost the same as Enzalutamide But does not have the fatigue problem for most people. You might approach that option with your doctor.

I’ve been on ADT for nine years. I’ve not had a fatigue problem, but I walk on the track across the street twice a day every day for about a mile each time. I also try to get to the gym three times a week. After I go to the gym, I jog the track after about a half hour rest.

One thing I have found is that electrolytes makes a major difference in energy. A couple of weeks ago I started drinking a liter of electrolytes Before going to the track in the morning at 11. If I don’t drink the electrolytes, I can jog about halfway around the track and then I have to walk halfway because I don’t have enough energy. If I drink the electrolytes, I can jog the whole way around the track for a mile in the morning and the afternoon just from having that electrolyte drink in the morning.

I was making the WHO version of electrolytes, which is a liter of water Two level teaspoons of sugar and a level 1/2 teaspoon of salt. I used tapwater and can drink it down in less than five minutes. If I refrigerate it, it takes forever to drink.

Just something to consider. Plenty of companies make electrolyte drinks you could just buy.

I find exercise to be great for my mental health but have had to adjust expectations: I cannot ride a bike nearly as far, nor work in the yard or house as much without serious breaks and frequent “blah” days when I just can’t get going and sit and read and nap. I also had shoulder surgery and then bilateral knee surgery (injury, not replacements) and have not been able to adequately rehab and get back to reasonable function. I have metastatic disease and am on Lupron + Daralutamide (Nubeqa)
