Edema/Pleural Effusion, rare but known Side Effect from Docetaxel

Apparently, edema is a less common, but known, side effect from Docetaxel, especially cumulatively - this was our experience. After round #4., during the heat, my hub noticed some ankle swelling. He had that in the past in the heat after being on feet a couple of times.

After Round#5, it got worse and it was noted at doctors appt pre infusion. He also started coughing more. But given that his PCa metastasis is lung-only, we suspected irritation and cough would be part of the deal. He also noticed his eyes watered like crazy. PA suggested drops for dry eye.

After Round #6, it was worse - to the point where we got compression socks…and cough got more frequent. And more leaking from eyes and nose. PA listened to his lungs pre-infusion and said they sounded good and said the loss of nasal hair impacts watery nose….

One month after finishing 6 rounds of Docetaxel + Carboplatin, we were scheduled for a follow-up PSMA PET and labs at the COE we have been returning to every 3 mos - it’s a distance. The scan showed ‘large bilateral pleural effusion’ and the images are wild - about 1/3 X 1/2 the lung volume is fluid and lungs are a bit collapsed (fluid is in the pleural sac surrounding). We presented with a fire to put out and he had a liter drained from each side.

Now we know about ‘capillary leak syndrome’ that can be a side effect of docetaxel. It’s rare, but sharing as rare happens to some. Prostate cancer is a real bugger. One thing I will forewarn about chemo - when you finish all rounds and you think you are done, there is recovery that they say takes 2 mos for every month of chemo. So ring that bell and then be patient!

I am sooooo sorry 😢 that it happened to your husband, how scary it must have been 😩. Oh my, how is he doing today ? That is just awful and I do not understand doctors if it is a known side-effect why was it so hard to order at least a basic x-ray 🤕 ??? *sigh

Thank you for the hugs - I was very glad we were at the COE to have it discovered and addressed. Actually, it was kind of insidious - we had no thought of lungs holding fluid - it was a surprise. Looking forward to chemo recovery now.

Thank you for posting this. I'm not on chemo but have already been told to expect it in my future. This would be one of those side effects I would never hear of otherwise.

When you research you will find its rather uncommon, but perhaps more likely with my hub because of the mets beibg in his lung vs other locations. So maybe its rare UNLESS the lungs are already compromised. Ankle swelling is more the ‘norm’ with this side effect.

Just fyi, my husband had no edema or pleural side effects with docetaxel. But good to be aware of the possibility -- just don't assume it is an inevitable side effect. Good luck to you.
