Dr taking me off Prolia to start Evenity

My Dr is taking me off Prolia to start Evenity as I am continuing to have compression fractures. I have been on Prolia for 18 months. Does anyone have experience with this crossover?

mouse60, Prolia to Evenity is an unusual sequence, primarily because of the rebound effect when stopping Prolia. My question would be Does Prolia cause rebound if the medication hasn't been effective?
I'm not a medical person, but it occurs to me that rebound is cause by the mechanism of Prolia and not by it's effectiveness.
When you quit Prolia all the osteoclasts that have been prevented from finishing development will complete their development and get to work removing your bone.
Evenity prevents the initiation of new osteoclasts, but won't affect the osteoclasts collected by Prolia.
I find the prospect frightening, but talk to your endocrinologist. There are many things that I don't know.
From reading, my understanding that there is no medications that can adequately address the massive increase in osteoclasts released after Prolia. And even Reclasts is sometimes inadequate.
I would be inclined to continue Prolia and add Forteo.
I'm so sorry that you are continuing to fracture. Did you have any bone markers to warn you about Prolia's effectiveness?

I've not had any tests that would lead me to believe that Prolia is not being of benefit except for 2 more compression fractures of my T1 and T3

It seems to that the goal of treatment is not to reach certain values in blood tests but to prevent fractures.

njhornung, I completely agree that the goal of treatment is not to reach values in blood test, but to prevent fractures.
The sad truth is that for all of these medications there are patients for whom the drug is not effective. So, the purpose of the serum bone markers is tell the patient the the drug is or is not working. A test before and a test one or two months after beginning a drug treatment can prevent fractures for these patients. To be on the verge of fracture and to remain so for 18 month or 2 years on a medication that is ineffective puts a patient for 18 months to two years of additional risk, not to speak of the often endured side effects is not advantageous in any way, even financially.

Njhornung, well said,
though maybe because we don't have other guides. bone markers, dexas, clinical trials, statistics— just numbers. We can be losing spicules like mad and still feel great.

I don't have experience with this specific scenario but my understanding is you need a biphosphonate (antiresorptive) to relay off prolia and prevent the prolia rebound effect of spontaneous fractures. That would be fosamax, reclast or similar. Evenity is primarily an anabolic, although it also has "some" antiresorptive effects. I wouldn't trust it to prevent rebound. Ask your provider specifically about why Evenity and not a true antiresorptive. Maybe you can't take a biphosphonate for other reasons? (In which case you'd be in the "prolia trap"). If it was me I would ask for bone markers, reclast, and continued monitoring to make sure you're out of the rebound danger zone before transitioning to Evenity or another anabolic. Good luck!

I don't have any direct experience transitioning from Prolia to Evenity but wanted to throw in a conversation I had with my endocrinologist. Each time I went in for a Prolia shot the doctor would ask if I had any falls. Sometimes I'd say, "yes." (tripping over the dog or grandkid toys) If my answer was, "yes," then she'd ask if I broken any bones. I'd always answer, "no." But then one time I had a question for her. I asked, "If I had broken a bone, then what would happen?" Her answer was that she would have to take me off Prolia and would then probably put me on Evenity.
