Cancer?? 2 thyroid nodules: Help understanding ultrasound results

Looking forward to someone being able to help me…

I had a thyroid ultrasound the other day and I will post some of the results. I am confused on all of it…
My doctor wrote that same night that it was concerning in order to find needle biopsy, which I don’t have until next week…

Backstory is they found two nodules in a CT scan I was having for something else so I only did the ultrasound as a precaution. I didn’t think anything would come of it so I’m very taken back. I did have one now Jewel 10 years ago that they biopsied and it was fine.

IMPRESSION:
1. High suspicion 2.1 cm nodule in the mid right thyroid lobe.
2. Intermediate suspicion 1.6 cm nodule in the upper right thyroid lobe.

EXAM: US THYROID

COMPARISON: CT Head neck angiogram 11/20/2024

FINDINGS:
The right thyroid lobe measures: 2.1 cm x 2.3 cm x 5.9 cm
The left thyroid lobe measures: 1.4 cmx1.0 cmx2.8 cm
The isthmus measures: 2.9 mm in AP diameter.

The thyroid parenchyma appears: Homogenous and mildly hyperemic with two nodules as detailed below.

A nodule in the mid right thyroid lobe measures 1.6 x 1.9 x 2.1 cm and has the following features:
* composition: solid (1)
* echogenicity: Predominantly isoechoic although there are a few areas hypoechoic (1)
* shape: taller than wide (1)
* margins: smooth margins (0)
* echogenic foci: no echogenic foci (0).
The Mayo ultrasound score is 3. Based on the AskMayoExpert Thyroid Nodule Care Process Model, the nodule has high suspicion for malignancy (>20%).

A nodule in the upper right thyroid lobe measures 1.1 x 1.3 x 1.6 cm and has the following features:
* composition: mixed cystic/solid (0)
* echogenicity: isoechoic (0)
* shape: not taller than wide (0)
* margins: smooth margins (0)
* echogenic foci: multiple punctate echogenic foci (2).
The Mayo ultrasound score is 2. Based on the AskMayoExpert Thyroid Nodule Care Process Model, the nodule has intermediate suspicion for malignancy (5-20%).

I read this and thought “I have cancer”. Now after research it says the rating scale tr 1-5 a 2 and 3 is not that high but looking at the report it says highly suspicious and intermediate suspicion…

It seems like you may need to have a lobectomy to remove the suspicious nodules and send them to pathology for examination.

I had a suspicious 2.5 cm nodule found on my right lobe incidentally when testing for something else. I had the biopsy and microscopic cytology and it was indeterminate so I decided to get it removed. I was in my early 50s and my sister had her full thyroid removed at the age of 27 due to thyroid cancer so I didn’t want to take a chance leaving it in there.

The pathology came back that the 2.5 cm nodule was a different makeup and not cancer but they did find a micro cancer elsewhere in the lobe.

Do you have any family history of thyroid cancer or Hashimoto’s thyroiditis? Have you had your thyroid bloodwork done for Hashimoto’s TPO autoantibodies?

Continued…I have to keep monitoring my left lobe due to a macro calcification and they even do ultrasound on the right side where the lobe used to be to monitor for any regrowth of thyroid tissue (wasn’t aware it can grow back).

I now take levothyroxine 100 mcg daily due to hypothyroidism.

My cousin had thyroid cancer and had the thyroid removed and then had a regrowth years and had to have that removed as well.

Thyroid blood work has always been normal…

@gilbie5, have you had a chance to meet with your doctor to discuss the results of the ultrasound in the meantime? What did you find out?

I was very nervous when my two nodules were 1.3 and 1.5.
My doctor told me over 1.5 they do fine needle tests.
Turns out I had a cancer called Papillary and it is very slow growing and not a very aggressive cancer. They took out the left side of my thyroid. The wait for results was a bit nerve racking. My margins were good and that is the best one could hope for. My calcium numbers were good.
I did have a scare on a follow up reading but turned out to be nothing. Sounds like your medical team is on this.
