Barretts at 24 Years Old

Hello, I am 24 year old female and was diagnosed with GERD, Barrett's esophagus without dysplasia (distal end), mild chronic gastritis (antral), and esophagitis from an endoscopy. I have been struggling with this since August 2024. I am already on the slim side but I used to weigh 103 lbs, now 88 lbs. Below are my symptoms, some tests I have done, medications I took/currently taking, natural remedies I am trying, and my diet/water intake. Any advice, opinions, or recommendations would be greatly appreciated! Thank you!

Symptoms:
Stomach pain/pinching/itching at bottom of stomach
Nausea
Early fullness
Burning in upper right abdomen area that comes and goes that lasts a few seconds
Tingling sensations on my back (the sensation moves around a lot but mostly upper back)
Chest pain
Lump in throat feeling
Chronic constipation
Regurgitation of liquid even when sleeping upright
Amenorrhea (haven't had period in 3 months - probably from under-eating)

Tests:
CT Scans: everything normal, mild thickening of distal esophagus

Ultrasound: everything normal, no signs of any problems with liver, gallbladder, pancreas

Endoscopy:
Esophagus, Distal:
Barrett's mucosa negative for dysplasia, minimal esophageal reactive changes consistent with gastroesophageal reflux, no evidence of eosinophilic esophagitis
Gastric, Antrum:
Mild chronic gastritis
No h. Pylori
Small bowel:
Unremarkable duodenal mucosa
No evidence of celiacs

Stool: No infectious bacteria, parasites, or white blood cells

SIBO: Negative

Vitamin Deficiency Test:
Vitamin D (Low)
Vitamin A (Slightly low)
Magnesium (Good)
Iron (Good)

Medications
Pantropazole 40mg (didn't work)
Omeprazole 40mg (didn't work)
Famotidine 20mg
Sulcrafate oral suspension 1g/10mL (currently taking 2x a day - 5mL half dose because constipation)
Gaviscon (Taken as needed for flare ups)
PPIs do not seem to work for me. They make my acid reflux worse.

Natural Remedies
Aloe vera in the morning (eating inner gel)
Arrowroot powder water
Artichoke water/tea
Chamomile tea
Slippery elm tea
Dgl licorice chewable tablets
Cabbage juice/boiled cabbage

Diet (all small portions)
Breakfast (8am): Veggie soup puree (carrots, cauliflower, green beans, beetroot), steel cut oatmeal, boiled chicken
Snack (11:30am): Banana, rice cake
Lunch (1:30pm): Veggie soup puree, chicken, soft white rice, half avocado
Snack (4:45pm): Banana
Dinner (7pm): Rice porridge (congee), salmon, spinach or boiled cabbage
No gluten, dairy, soy, red meat, eggs, or processed foods except for rice cake, crackers, fig bars. All bland diet.
Chewing slowly/thoroughly

Water Intake:
80-100 fluid oz a day
I drink water mainly 15-20 minutes before before and 30-45 minutes after meals, never during.
Constant dry mouth

Exercise:
Light walking after meals for 45 minutes

The Gaviscon should be the one with Alginate (it's called Gaviscon Advance). Otherwise, it's just calcium like TUMS. Too much of it I think could give constipation. But, the Gaviscon with alginate is important for you, to protect your esophagus.

Sorry to hear that you’re suffering with reflux and BE.
You already know to cut out the acid, no smoking or drinking alcohol or soda. I got BE at age 63 and did a strict diet. I was on Pantoprazole which got rid of Barrett’s on my last 2 scopes. It was not to be seen. Don’t eat 3-4 hours before bedtime. Only water. I drank unsweetened almond milk which is non dairy and alkaline. I also drank water that has a ph of 9.5 which Kroger grocery store sells. I don’t know what else to tell you. You can search Barrett’s esophagus on this site and read all the comments about it. Good luck and hope you find an answer.

Wow you are doing so much already. Can you do more testing to see if you have, for instance a hiatal hernia (an endoscopy should have seen whether there was one but sometimes it gets missed. I bring that up because you noted chest pain). Some people rave about functional doctors (who usually don't take insurance), tho my local one didn't do anything for me. You are doing so much right already, it must be very frustrating for you.

For dry mouth, I found that chewing sugarless gum boosted my saliva production and many times keeps me from suffering chest pains while exercising.

If you increase intake of Gaviscon Advance before suffering an episode, will that help to prevent reflux? For instance, after eating/drinking and before going to bed . Are you taking Gaviscon Advance, the UK version? That is the best version. Available on Amazon. I used to take GA after meals and before bed all the time and am now able to dial that back. I mostly use it now when I go off the proper diet.

When do you go to bed? 7 p.m. dinner may be too late.

ENT Jonathan Aviv has info on different SM platforms, and there is a FB group that offers a ton of recipes for low acid, whole food diet based on Aviv's work. Their name is acidwatchers diet warriors, or something like that. Please keep us posted on your progress and keep pressing doctors to keep exploring/troubleshooting. I wish you well.

I have been taking Omeprazole for 13 years with little effect, slightly better than nothing, but last year I was told to take it at least 30 minutes or more before a meal, it started working then, 60 mg per day. When I get into trouble after eating, then I take acidex, similar to gaviscon. Had all the tests possible and returned negative to everything but SIBO and undecided on Barretts, it seems to depend on which "specialist " sees me! I have very similar symptoms to you and have been labeled as IBS because they don't know and it sounds good to them! Subsequently 6 months ago I was told I have Dysautonomia.
I have tried all manner of medications and remedies, dropped most of them all but for Omeprazole, laxatives Vitamin D3 and B12 supplements, both vital for your health and a prescription multivitamin, but don't overdo the vitamins as they can be every bit as dangerous as not enough, best taken under Dr's supervision.
The amount of food and fluid I can consume is my biggest problem. I have to limit the daily amount as dysautonomia has slowed my digestion significantly and strows me into SIBO periodically.
I essentially live on 4 slices of wholemeal bread, 2 slices of shaved ham and min 6 cups of tea per day. What you are eating would be a veritable feast for me!
Once I was diagnosed with Dysautonomia all the professionals lost interest in helping me. I do understand why as there is no cure or treatment know, yet. So I eat what I can despite it being "not good " for me as I don't have the reaction to some foods I'm supposed to have. The bread is not good for me, neither is the occasional raw onion I might sub out for the ham, and I'm not supposed to eat the ham either, but I tolerate it well. Onion happens to be both a prebiotic and a digestive. I find small amounts of Ginger beneficial as well.
Keep trying variations and stick with what you find works for you, we are all different so the hard and fast Fodmap style diet doesn't work for everybody. I'm usually open to try most thing because you never know when something my help you when it is not supposed to.
The biggest help has been to keep myself occupied and my thoughts away from my health issues, I don't get so much anxiety and forget the starvation pangs for a while.
