Anyone had a spike in osteocalcin while on Tymlos?
Hi to all,
In preparation for my upcoming visit with my endocrinologist, I just had a battery of blood tests, and I got one unexpected result that was so out of whack that I wondered whether anyone else had experienced something similar.
Basically everything was fine, including blood serum calcium, except for three tests. Two liver enzymes were slightly out of range, one a drop low, one a tad high, but neither by much. The one that floored me was the reading for osteocalcin, a marker for bone turnover. It’s always been well within range. I started Tymlos last January, and on two tests last year the level in Feb was 24 ng/mg and in Aug it was down to 13.1, which is surprisingly low considering how Tymlos works.
Last week the level had spiked to 59.9 ng/mL (the normal range is 5.0-29.5). I emailed the doctor, who said that osteocalcin does rise in people on Tymlos and that she wasn’t concerned but would monitor the level.
Tymlos has dramatically improved my bone density in my spine, according to my recent bone density scan. Which is great. But Dr. Google has failed me utterly in trying to figure this out, so I thought I’d see if anyone here has some wisdom to share.
Thanks.

There are 3 bone turnover markers used nowadays to reflect osteoblasts' activities: bone specific alkaline phosphatase (BSAP), osteocalcin (OC) and P1NP -Procollagen Type I Intact N Terminal Propeptide. In human studies,, levels of OC correlate with bone formation rates. So it's great that you are having a high OC while on tymlos!
Did your endo test other bone markers?
