Any hope for aggressive metastatic prostate cancer?

My husband, 60, just got an MRI report that stated aggressive metastatic prostate carcinoma >15 that spread to bladder and anus walls. We have an appointment with the urologist in 3 days. This feels so hopeless. Symptoms have been difficulty urinating mostly. PSA ranging 5-7 over the past month. IS THERE HOPE?

Unfortunately, no one knows how long any of us has — all we can do is hope for the best, and support that hope with everything medical science can provide. New PCa treatments are emerging every year, so surviving just one year longer could get you lots more years we can't anticipate yet.

Statistics for overall survival are very broad brushes that may or may not depict your unique situation. There's an old joke that if you ask a statistician, they'll tell you that the average human has one ovary and one testicle, but there's a lot of wisdom hiding behind that joke for those of us who spend too long staring at cancer stats.

When I was first diagnosed with stage 4 PCa in 2021, I interpreted the stats my oncology team gave me —to expect progression in 18-24 months (which didn't happen), and 5–7 years remaining life (which they've since retracted) — as a kind of death sentence. Now I understand that they're just helping us to understand what could happen, not telling us what *will* happen.

Just got my PSA It is 96.1 down from 181.6 so the 2 chemo's got it down from PSA flair The Zytega is best if one had chemo doxcitel and it failed versus Darolutamide and its approved

In Feb I do a PSMA PET Scan and hopefully get approved for 6 injections of LU 177 All 10K each covered by Ontario

I was on Xtandi then Radium 223 each for a year and did 2 chemo's with Doxetel which caused fluid in lungs now since gone after 6 weeks of Prednisone . Prostate radiated in 2017 Spread to spinal bones only in 2020. Now spots in T 10 and L4 to 5. Just had a slip and fall on sidewalk ice at bank last week and fractured my L1
Plan is to do a CT-PSMA scan in Feb and see if I qualify for LU 177 now approved as standard of care in Ontario
Advise appreciated How long do I have??

Metatastic prostate cancer getting worse soon my spine on my thoracic don’t know how big yet got video conference on Thursday with radiation Dr just wondering has anyone had radiation to this area if so what should I expect thanks

In 2020 had L2 radiated 3 cm or 1 inch cancer out Worked fine Go for it

My husband just completed 4 weeks radiation including on areas you mentioned. Urination issues but really no other side effects … thankfully

My husband had similar diagnosis. Metastatic prostate cancer, PSA 5, has had 4 weeks radiation and first hormone injection. We know cancer has spread to at least 3 bones. What should we expect…..prognosis?

I would encourage watching the first presenter in the mid-year PCRI conference and here is the link https://pcri.org/2023-myu she does a very good job to put the entire topic of treatment for prostate cancer into context. Look at 29:00 for the chart of drugs and see if you are familiar with them all, if you aren't then get educated where it is appropriate. Look at 1:07:00 for the health team and make sure that you have names in each box for your health team. Time is on your side and time is well spent being educated. It can be overwhelming at times. Keep the Faith!

Yes, there is hope for people diagnosed with metastatic prostate cancer. (Cancer that has spread beyond the Prostate.) News like this can be overwhelming and scary. You are doing the right thing by reaching out and trying to get help. Your husband’s diagnosis is serious but you are at the beginning of the journey, not near the end.

By now you have probably met with the Urologist and he has recommended treatment options. Most likely the first step is starting Androgen Deprivation Therapy (ADT) to slow the growth of the cancer. This will give you time to decide on your treatment options. I hope he also recommends a PSMA PetScan. This type of test is a full body scan that can detect cancer anywhere in the body with accurate detail.

Once you know the full extent of the disease it will be time to choose the best treatment options for your husband.

I was diagnosed with Prostate Cancer at the age of 48. Gleason 7 (3+4). I had Robotic Prostate Surgery to remove the gland. I am now 64 and I had a recurrence of cancer that spread to my bladder . Gleason 9 (4+5) I recently went on ADT (Eligard injections with Zytiga) and 37 treatments of Radiation Therapy.

I hope this helps you at this difficult time. Please post an update on his status.
