1.3cm Nodule discovered during routine x ray
I have a referral to see and Endocrinologist and will be contacted in 5 days though the UCSD system. The ultrasound shows a 1.3cm nodule along with a ACR TI-RADS 20127 of T4 (moderately suspicious) Mixed cystic solid 1pt, echogenicity: Isoechoic 1pt , Echogenic foci: Punctate calcifications 3pt , macrocalcifications 1pt. Of course I googled it to get a better understanding. Punctate calcifications is a bit worrisome and seems to be the biggest issue? I do have some symptoms. I have been unable to sleep at night, I have a new pain on the side of my neck, sensitive to heat, and a bit of depression. I don't know what to determine at this time. I'm awaiting contact from a refereed Endocrinologist. I am 68yrs old female and feeling pretty scared that it may be cancerous. I'm also having severe cervical neck issues and was scheduled for a Laminectomy with fusion but now it is on hold till I find out about my thyroid. I found lots of answers to my questions in the Spine support Group. Hoping to find some answers, direction and support now that I have a thyroid nodule that looks suspicious.

janagain, I'm not qualified to answer your question, but since I'm the only: I do have a small punctate benign nodule on my thyroid too small to biopsy. Nodules are often an indication of autoimmune disease (mine's Hashimoto's). While you have the worry and should insist on a needle biopsy, making sure they take a large enough sample to do somatic testing, you have two advantages. At the worst if it were to be (pray not) cancer, you are discovering it early. And they are now targeting the genetics mutations of the tumor itself. So good timing. You might be able to eliminate the tumor with radiofrequency. While you have to be suspicious, if you ask google whether Hashimoto's causes each of your symptoms, the answer is yes.
You might try to get a needle biopsy ordered in advance of the endocrinology appointment, since you may have to wait. Look at all the print on your imaging report and see if anyone has recommended biopsy. The you could probably get the interventional radiologist to perform or order one. I stress the importance of getting enough of a biopsy sample to have the genetics of the tumor itself tested.

I am in my mid 50s now and had cervical spine issues and had ACDF surgery in 2022. When paying my bill, I looked at my presurgery ct scan report and it showed a nodule that no one told me about. My sister had thyroid cancer in her 20s and had her whole thyroid removed.
I went to an endocrinologist and had ultrasound and fine needle aspiration and it was suspicious and was going to only be monitored. I wanted the lobe out since it was 2.5 cm as I was having swallowing and speaking issues (may also have been tied to my cervical spinal cord compression). I ended up going to Massachusetts General Hospital to see a new endocrinologist and surgeon. I had it removed and the large nodule was luckily not cancerous but they did find a micro cancer. I am now taking levothyroxine due to Hashimoto’s thyroiditis confirmed via pathology and post surgery hypothyroidism. I have a large macro calcification on my left lobe that needs to be monitored and may need it removed it it proves to be cancerous over time.
I just had my 2nd cervical ACDF surgery a month and a half ago and encourage you to not delay your cervical spine surgery, especially if you have spinal cord flattening/compression which is an injury to your spinal cord that can cause permanent damage. I would prioritize cervical spine over thyroid and you can have surgery that does not disrupt/disturb your thyroid. You can continue with the endocrinologist and if surgery is needed on your thyroid, they might be able to use the same scar for the incision (they did that for my first ACDF surgery and thyroid lobectomy but needed a new incision lower for my 2nd ACDF surgery due to scar tissue).

I'm going to call the office tomorrow and set up an appointment. I was going to ask about the timing on all this since I am having a very much needed laminectomy. Cord flattening and losing all muscle in arms, legs, fingers. I would have had a Laminectomy sooner but came down with a UTI. Then the thyroid nodule noted on and xray. That too has held up things. I agree the laminectomy should come first but now I think my doctor is taking 6 weeks off. Hopefully he left earlier than I am thinking and I can wait for him to return. Booking the surgery suite is backed up till December. I just don't have any more time to take care of this issue before it becomes permanent and may have already. I'm feeling lousy and I've got some symptoms I never realized it could be a symptom from the thyroid. I feel like I'm going to pass out at times. I had an MRI and got off the table, went out to the hallway and the hall started swirling and I leaned against the wall to hold on. The nurse came out and caught me. She gave me some water and I was ok after that. When I had the thyroid ultrasound I felt dizzy again. I've never had episodes like this before. I'm going to let them know I do not want to delay my laminectomy. Thanks for the advice.
Since I scored a 5 I am assuming they will needle biopsy the thyroid? Please be honest, is that score something I should worry about? The result of the Echogenic foci: Punctate calcifications (3 pt), macrocalcifications (1 pt) has me a bit worried. I feel the lump when I swallow but then again, I've had cervical issues and have spacers and that could be the lump that I'm feeling. I'm unable to sleep at night and I'm exhausted during the day. My neck is sore also. I'm going to ask about using the same scar , that would be really nice . Thanks for the advice. I've developed lots of scar tissue, bone spurs and scarring from surgeries. Another surgery I'm going to end up having also are my right and left thumbs. The bones in my thumbs are piling up on top of other bones. I will need surgery to correct that at some point. The body can only take so much. I hope I can get this all figured out and get my life back again. Thank you for listening. Your suggestions and ideas are very much welcome.
