Prolia/Forteo combination therapy?

Posted by amrh @amrh, Jul 10, 2023

I have been doing a deep dive into the sequencing of osteo meds and have seen studies indicating the benefit of combining Prolia and Forteo.
https://www.thelancet.com/journals/landia/article/PIIS2213-8587(19)30255-4/fulltext
"Combined treatment with teriparatide 40 μg and denosumab increases spine and hip BMD more than standard combination therapy. This large and rapid increase in bone mass suggest that this high dose regimen might provide a method of restoring skeletal integrity in patients with osteoporosis."
Does anyone have experience with this?

I am currently on both, Prolia for 5 months, one shot and 8 weeks on Forteo, daily shots of 20 ug. I also did some heavy research and did a telehealth consult with a bone specialist with 35 years experience. He recommended the regime and specifically stated that studies indicate added benefit using both. Thanks for the link I’ve been looking for it.

I read that and wish it had been an option in 2021 when I started Tymlos. Reclast afterward would have to be carefully timed since lingering Prolia can affect effectiveness., according to Dr. McCormick.

What happens if you need to discontinue one or both drugs?

The above cited article is from 2019. Has anyone seen more current similar data? Or being managed by a doctor using the same combo?

Though the combination results are amazing I would just be concerned with how you could differentiate if problems arose , which med was the cause and how one would proceed??? THOUGHTS??

What is ultimate plan. Stop Forteo after 24 months and continue on Prolia?

Did you go to Reclast after 24 months on Tymlos?

@connectalant my endo is at a major teaching hospital but I am the only patient in his practice doing Evenity after Tymlos. I have done 3 months so far. the last two only one out of two shots. I am deciding whether to continue Evenity or go to Reclast due to some side effects.

@amrh just want to note that the study in the link posted is evaluating 20mg vs 40mg Forteo (teriparatide) with Prolia (dsnosumab) given at the same time, starting 3 months after the Forteo started. This is based on the DATA study and is not evaluating the combination, which is in the DATA study, but evaluating the two doses of Forteo (the 40mg did better).

Ben Leder MD mentions the combo in his You Tube video on Combinations and Sequencing, and I believe Mccormick may mention it in his book too. Leder says that in some ways, Evenity accomplishes this combo of effects with one drug. And it is easier to get off of!
