I take progesterone, estradiol.0375 patch, testosterone pellet and vaginal estradiol cream. I've had a hysterectomy.Among other benefits I no longer have urine leaks.
And to answer your "why" question the money allocated for women's health research is a pittance! That is outrageous. I notice that few women are also getting testosterone as part of their bhrt treatment.
I am 76. I had a total over 30 years ago. My doctor ordered daily oral estrogen. I took that for at least ten years and all was well. Then my doctor took me off saying it could cause breast cancer. I went through the terrible cold turkey symptoms. Fast forward years until about 3 years ago when I developed vaginal atrophy which caused my labia to split into painful cuts. The drop in clinic gave me the estrogen cream and it worked, but I was concerned so I went to a GYN. She recommended rubbing the labia with coconut oil. I have done this ever since and it has cured the splitting. I have zero discomfort now. I am not sexually active and I don’t know if the coconut oil would be advised if I were sexually active.
