Excruciating pain when standing up after TKR, 18/10 weeks in

I am 18 weeks postop from my RTKR and 10 weeks from my LTKR. My ROM is great, my walking is great and my PT and surgeon say I’m doing great. BUT, it is excruciating when I go to sit, but mostly when I stand up after being seated.
I’m told it can take 6 months for this to improve.

Good evening @shar2205, welcome to Connect. It sounds like you are doing very well especially when you consider that you are dealing with two recoveries. Just so you know, I have also had TKRs in both knees. And my memory tells me that I too had similar post-surgery issues with the first TKR. That first one was 10 years ago and there were no pre-surgical exercises recommended. As a part of my trekking group, we walked about 3 miles a day in preparation for one of our 5-day treks across Ireland or down the coast of California. (We usually walked about 10-14 miles a day.)

I recall that after my first TKR, it was about 6 months before I could get up in the morning and walk without any discomfort to the bathroom. This last time I went through about 6 weeks of pre-surgery exercises to strengthen my leg in preparation for the recovery. My only real post-surgery discomfort was for about 3 weeks. I seemed to have quickly moved from walking around the house 6 times a day to walking along the Mississippi River for at least an hour.

Have you talked with your PT or Surgeon? Where exactly is the excruciating pain when you go to sit or stand....in the knee, in your thigh? Are you sure you do not have any post-surgery scar tissue that has built up? Or an IT band that needs some work by your PT.

My recent recovery was also made easier by seeing my MFR (Myofascial Release) therapist twice a week. Another inquiry? Are you climbing stairs or walking up and downhill yet? Do you have or have you tried any topical pain medication, either Medical Cannabis or another recommended option?

These are just some suggestions that may be worth investigating. And remember if it does take 6 months, it will be less painful in month 5 and then again in month 4, assuming that the treatment you choose works for you.

I had hip issues after my TKR and I only did my right. Going from a sit to stand does hurt sometimes but I never experienced excruciating pain. I know in PT I did many sit to stands which were difficult for me. I still have scar tissue in my quad tendon which causes a pulling on my knee like something is over it as well as pain like a baker's cyst (which I apparently do not have). My surgeon told me from 6 months on I was find but I knew something was wrong and it was a sport medicine Dr. that did an ultrasound and looked. So, here I am 16 months out going back to PT to break up this scar tissue and I still have inflammation. You're still early on but by month 5-6 you should start seeing improvement and it not pursue it even if your Dr. says it's all good. In PT with me now they are doing cupping, MFR, and a procedure called Iontopheresis which is helping me but I am still very sore after again. But it's necessary and it does hurt but the end result is what I focus on. Best of luck and remember to always pursue if you feel something is not right.

I am 4 weeks after TKR left, and experience this pain and stiffness after sitting as well for a short time.
But it’s getting better every day and I think I know why.
From my first day on, I did my exercises at home, daily 3 times with icing afterwards. After the 2. Week I started doing them 2 times a day with icing, additional PT 2 times a week.
I can bend my knee right now 126 degree and my goal is to get it up to 130, what is the top of the line.
Yes there were tears in the beginning, a lot of pain when I exercised but I did it anyway.
And your day is filled of exercising and icing, what is not fun but I am happy that I did it anyway.
This is just my experience, I have already 2 shoulder replacements, TKR is different!
