I just found one of my notes. Go to this link, scroll down and hit download:

Then give that information to your pharmacist. They will want the RxBIN#, PCN#, Group# and Identification.
My copay went from $777.76/month to $4.00/month.

It’s possible this will depend on what insurance you have but it or your pharmacist will tell you if you are ineligible for some reason.

I as well have Blue Shield commercial insurance.In my phone call last year I was also told about the $4 per month , but the catch was it would only be usable till you hit the $9,000 mark , which only brought me to 4 ish months , after that would have to pay full amount on my own that insurance charged for it which came out to about 18,000 for the rest of the year … however could do the same thing the second year.
BUT now that I’m getting close to maybe pulling the trigger on actually taking Forteo I called my ins guy last week and he was very helpful and talked with Blue Shield and though I have about a 500 per month charge he said when I meet my deductible that then it would be essentially free. Your plan is probably different than mine, and he also said to know the real figures your authorization first needs to go through - that’s when the real numbers you’re dealing with are revealed. GOOD LUCK TO YOU AND ALL OTHERS MUDDLING THROUGH THIS!!!
