Anyone have experience with anal intraepithelial neoplasia (AIN)?

I have been recently diagnosed with AMI3. This was found during a routine gyn exam. I had a punch biopsy first with a result of HPV 16. I had further exam under anesthesia with biopsy which showed AMI3.
My surgeon recommend exam every 3 months, and Aldara cream. The margins were not clear with the biopsy. I am 64 female, with one partner for 30 years. No anal sex, just trying to figure this out. Is there any other treatment options??
I am scheduled for another exam in November to see if he can get the margins clear, although the surgeon says it is doubtful.

There may be other treatment options available to you. If I were in your situation, I would want to better understand why the margins are not clear and what can be done. How long ago was the biopsy? Why the wait? Have you considered getting a second opinion?

I’m sorry to hear that you’re going though this. I’ve just come out the other side of a battle with anal cancer. Mine was squamous cell caused by HPV. You can have the virus for years and not know it. It sounds like you owe it to yourself to get another opinion. I’m not a doctor, but not getting clear margins on a biopsy sounds odd. Do some research on your own and find a doctor with lots of experience in dealing with this.

I am cancer free after having anal cancer diagnosis in March of this year! It was hell but after surgery to remove the cancer, chemo and 35 rounds of radiation I am cancer free! Such a good feeling! I still have issues with bowel movements as my muscle was damaged during surgery but otherwise I am going back to the gym and living a normal life! Praying for you! Don’t wait to get a second opinion if you feel you need one!

Hi, I'm 53 years old and last year I was diagnosed with AIN 1 this year it has progressed to AIN 3. I'm in so much pain and discomfort almost every single day, itching, burning etc. I'm scheduled to undergo laser excision surgery next week. This thing seems to be moving quickly. Can anyone tell me if AIN 3 is considered early stage anal cancer...Has anyone undergone this type of surgery? How painful is it? What was your recovery time.

This is not cancer but rather abnormal squamous cells which might develop into cancer.You will need treatment because the abnormal cells are less likely to get better on their own. They might develop into anal cancer, but the risk of them changing to a cancer is very small. Your doctor will discuss the best treatment option for you. Treatments might include:

laser to destroy the cells (laser ablation)
surgery
skin creams such as imiquimod or 5FU.

Talk to your doctor or oncologist about the specific treatment but chances are it won't be that serious but uncomfortable while the healing post surgery occurs

Hi Roy,
Thank you so much. Yes, I'll be receiving laser ablation surgery in a few days. I'm a little nervous but I'm ready. I've heard lots of stories on issues surrounding recurrent. I pray that's not my case. Please let the pain be minimal.

Thank you for your kind words. I will try to remember your advice. The doctor wants surgery first as the location and size is becoming a bit of a problem. It's a bit disconcerting as research states surgery is a radical step to begin. The doctor wouldn't discuss the AIN part as she's a surgeon, not a oncologist.

Stop don't jump into surgery as you cannot go back & replace the sphintor. Surgeons just want to cut they want a job you have to live with the after effects not them A radical surgery & a bag for the rest of what they call life I too have problems with scheduling and what is going on They tried to send stuff via cellphone after beating their eardrums to death I do not have cellphone coverage where I live They finally got the message most of the time but then at times wonder why I did not come for such & such appt. I ask how did you send the message o well we did it to your cellphone well dummy read my chart
And also I live in a very remote area & the experts of the town would love to know about my condition and just about stop you from coming into town. They are that closed minded Gossip is much better to talk about than the truth The only person that knows about this is my husband and I banned him from telling anybody especially his sister who is head gossip of the town
