So, Dr Shallenberger appears to have quite a trail ...lots more than these, but apparently he has been repeatedly investigated?

There is a lot to look at. The most recent I found was filed by the Nevada Board of Medical Examiners dated April 2023. There is no resolution on this one that I could find. It shows up this way in the Google search:
