Medical test options for post menopause spotting, cramps
I'm 66 and had vaginal spotting for four days in June. Did not see a doctor until July due to caregiver support for family member. Met with my gyn PA for physical exam. She ordered an ultrasound which showed previous fibroid was unchanged in size (roughly 1.5") and my uterine lining increased from 3cm to 5cm. She has suggested a biopsy but that I meet with one of the doctors in the office to discuss options. She said some women wait until the uterine lining is 10 cm or greater to do anything. I'm not comfortable with that, specifically because the cramping is getting worse.
My appointment with the doctor is August 31. My situation is a bit more complicated since my vaginal has narrowed making an in office biopsy more difficult. The PA mentioned I might need to go to the hospital for the biopsy.
Not having the benefit of meeting with a doctor, I'm leaning towards a biopsy. What is the difference between dr office vs hospital biopsy?
Does anyone else have a narrow vaginal with same symptoms? If so, how was this handled for you? I realize everyone is different. I'm just trying to get an idea of what I should be considering and questions to ask the doctor.
Also, my cramping seems to be getting worse. What does this mean? Not sure I can wait until August 31 with this pain. I am on the wait list to see the doctor earlier if there is a cancellation.

@leeosteo My lining was only 5mm (.5cm), and I had an endometrioid adenocarcinoma that had grown over 60% into the myometrial wall. I only had slight pink spotting two times, no other symptoms, but thankfully went to gyn right away. Even though radiologist wrote “ endrometrial carcinoma would be unlikely…” my gyn took no chances and did office biopsy.
may27 I'm definitely going for a biopsy based on input from this forum. In your case, what was the next step for you? Did you have a d&c? or partial hysterectomy? More importantly, how are you doing now? I hope you had a good outcome. Thanks so much for sharing your experience. I meet with the doctor tomorrow to discuss options.
@leeosteo So glad you will be sure to have a biopsy. Mine was done in the office. Diagnosed (Grade 1 Endometriod Adenocarcinoma) June 2, Robot-assisted laparoscopic total hysterectomy (incl ovaries and tubes) July 6th.
Lymph nodes clear, peritoneal washing clear, but there was lymphovascular invasion of tumor into 60% of wall, so Brachytherapy radiation recommended. Stage 1b1. Feeling fine, recovery from surgery going very well. Good luck tomorrow, keep us informed, will be thinking of you.

I had an in-office ultrasound and endometrial biopsy after a few days of spotting in 12/22. A uterine polyp was removed and my endometrial lining was measured at 7mm, but no cancer was found. A year later (almost to the day) the spotting recurred. This time, my GYN scheduled a hysteroscopy d&c a month later. Another polyp was removed. A few days later my GYN called to say that it was cancerous...adenocarcinoma. That was frightening for me because I had lost one older sister to that same diagnosis (a recurrence was missed until it had metastasized) and another older sister had just undergone surgery for the same diagnosis in May 2022. I underwent a total hysterectomy, bilateral oophorectomy and bilateral sentinel node biopsies in late February 2022. I chose to go to the Mayo Clinic in Rochester for this, as I was having trouble getting calls back from the local hospitals/GYNONC's I was referred to initially. I'm SO glad I did this and didn't wait. As a result, we learned that the hysteroscopy and d&c had reached and removed all the cancerous tissue before I ever got to the hysterectomy. The only way to have found that out, though, was the surgery, sentinel node biopsies and pelvic washings, which for me, were clear. I can't echo the others' sentiments in celebrating that you were able to get the hysteroscopy scheduled yesterday, sooner rather than later. I did not have much pain after the hysteroscopy at all; I just gave myself a day to rest and got on with life and work. I do hope that you are recovering comfortably after your procedure and that good news comes to you. If not, we are all here, rooting for you.
All, my update. Met with the GYN on Monday. We discussed my history and symptoms at great length. On the GYN recommendation, I will be scheduled for hysteroscopy with D&C sometime within the next few weeks.. unscheduled at this time. I appreciate everyone's input and willingness to share their experiences. Will keep you posted as to how it goes. Thanks!
@leeosteo Oh, this is so good because it’s what you wanted. It sounds like « at great length » means that your gynecologist took plenty of time with you and listened to your symptoms and your worries. Once appointments were scheduled I felt less anxious because there was something concrete in place. How are you feeling this morning? When you have the actual date for the D & C and hysteroscopy will you let me know?

Being an RN, I know how some of the Dr.s you might see, may seem very lacadaisical, and not interested in being your Medical partner and diagnostician, and act quickly to get the tests you need, quick results, and surgical treatment if needed.
As women, we know our bodies. We always have that gut feeling when something isn't right. Fight for what you know you need. Like the biopsy that was necessary, now, not next year. Or the appt. that they made you 2 months from now, when you need it NOW.
This happened to me in the beginning. I told the receptionist at the gyn office that I know I have cancer (after bleeding), and she will make me an appointment NOW. Usually they just look at a schedule and see she is booked. But I know that most Dr.'s have a few slots in their daily appts for emergencies. and they will see you.
I've even gone to the office if I don't get results, and make sure the Dr. will let me talk to her in between patient rooms. That usually works.! Always be proactive for ourselves. My mother taught me that.
